Το σύστημα αποζημίωσης  των οικογενειακών ιατρών στις ΗΠΑ  είναι επιστήμη.
Πέραν το Medicare , κάθε ιδιωτική ασφαλιστική έχει το δικό του σύστημα αποζημίωσης.
Αρχικά η αμοιβή καθορίζονταν από έναν αλγόριθμο Relative value units (RVUs)και επιπλέον υπηρεσίες χρεώνονταν έξτρα  όπως διαμορφώθηκε από το Omnibus Budget Reconciliation Act of 1989 & Balanced Budget Act of 1997  .

Το Affordable Care Act ("ObamaCare") το 2010 έφερε το Subsidized Coverage (το οποίο απέτυχε ) και τέλος  ήρθε το MACRA .Medicare Access and CHIP Reauthorization Act of 2015 (MACRA), όπου δίνει έμφαση σταδιακά στην ποιότητα τού έργου κι όχι στον όγκο.

Physician Reimbursement Under Medicare

The Patient's Perspective
Providing health care insurance for 39 million aged and disabled persons, Medicare cost an estimated $222 billion in 2000, a 3% decline over 1999.[3] Of this $222 billion, $33.1 billion (15%) was spent on physician services.[3] Medicare has two parts: Parts A (hospital insurance) and B (medical insurance). Part A is financed primarily by payroll taxes (1.45% of a wage paid by employer combined with 1.45% paid by employee) and is premium free for nearly all beneficiaries. Approximately 25% of Part B is financed by monthly premiums ($50 per month) paid by beneficiaries and the remainder from general federal revenues.
A person is eligible for Medicare Part A if he/she is 65 years or older and eligible for any type of monthly Social Security benefit. When a person enrolls in Part A of Medicare, he/she is automatically enrolled in Part B unless that person declines. If the individual enrolls in Part B, the premium is $50 per month and is deducted automatically from monthly Social Security benefits.
When Medicare beneficiaries are first admitted to the hospital, they pay an initial deductible of $792. Beginning on the 61st through the 90th day, they pay $198 per day in coinsurance. After the 90th day, they pay $396 per day for as many as 60 additional "lifetime reserve" days or can opt to pay the entire charge and continue to bank the 60 lifetime days. If there are more than 60 days between the discharge and a subsequent readmission, a new benefit period starts with the same $792 deductible and coinsurance provisions.
Medicare beneficiaries may be eligible for skilled nursing facility benefits if hospitalized for more than 3 days and if the skilled nursing facility admission begins within 30 days of the hospital discharge. In that case, the beneficiary owes nothing for the first 20 days and $99 per day for the next 80 days. There are no benefits after 100 days. Home health services are also covered if a beneficiary is confined at home with a 20% coinsurance payment for those services.
For Medicare Part B, the beneficiary is responsible for the first $100 of allowable charges per year. After that, Medicare pays for 80% of all physician-related charges and the beneficiary is responsible for the remaining 20%.
Recently, those eligible for Medicare have been given the option of enrolling in two other forms of Medicare. A person may opt for a Medicare managed care plan in which health care networks provide care for Medicare patients in return for a set monthly payment from Medi-care. The main difference in this plan from the original is that the risk of caring for the patient, regardless of the number or complexity of illnesses, is shifted from the government to the health care network. In addition, a Medi-care recipient may opt into a private fee-for-service plan offered by a private insurance company. Medicare pays the insurance company a set amount for each beneficiary per month, and the insurance company in turn charges the beneficiary an additional amount based on the amount of services the beneficiary consumes.
Finally, Medicare beneficiaries may choose from one of 10 standard Medigap plans that, for a monthly premium, will cover the "gaps" in Medicare coverage or provide additional benefits such as limited coverage of prescription drugs, at-home recovery, foreign travel, or preventive care.
Physician Payment
Since January 1, 1992, payment for physician services has been based on a fee schedule. The fee schedule also applies to chiropractors, podiatrists, optometrists, nurse practitioners, and physician assistants. The use of a fee schedule replaced the prior method of paying for "reasonable charges" because of wide geographic region-related variations in fees, a rapid rise in program payments, and the fact that payments frequently did not reflect the resources used and that physicians in different specialties could receive different payments for the same service. The new fee schedule is based on the "relative value" of the service and was recommended to the Congress by the Physician Payment Review Commission, a congressionally established advisory body. In September 1997, this committee was replaced by the Medicare Payment Advisory Commission, which is an independent federal body that advises Congress on Medicare including issues of physician payment. It is composed of physicians, nurses, osteopaths, clinical researchers, attorneys, and insurance company executives.
The fee that a physician is paid has three components: the relative value for the service, a GPCI, and a national dollar conversion factor. The relative value portion of this equation is composed of the following: 1) a physician work component that measures the time, skill, and intensity associated with the service provided -- this component accounts for 54.5% of a service's relative value; 2) a practice-related expense component that measures average practice expenses such as office rents and employee wages and that varies on a code-by-code basis depending on whether the service is performed in a facility or nonfacility setting -- this accounts for 42.3% of a service's relative value; and 3) malpractice expense component that reflects average insurance cost -- this accounts for 3.2% of a service's relative value.[2] The GPCI is designed to account for variations in the costs of practicing medicine. A separate GPCI is determined for each of the three component of the RVU and takes into consideration median hourly earnings of workers in the area, office rents, medical equipment and supplies, and other miscellaneous expenses. There are 90 GPCI areas nationwide. The conversion factor is a dollar figure that converts the geographically adjusted relative value for a service into a dollar payment amount. The conversion factor is updated yearly. The conversion factor for 2001 was $38.2581. The law specifies that physicians who provide covered services in any rural or urban Health Professional Shortage Area (there are 2,901 such areas in the United States) are entitled to an incentive payment of 10% more than what they would otherwise be paid under the fee schedule.[4]
The general formula for determining total RVUs is: [work RVU x work GPCI] + [practice expense RVU x practice expense GPCI] + [malpractice RVU x malpractice GPCI] = total RVU; payment = total RVU x conversion factor ($38.2581 for 2001). See Table 1 for several examples of payments in Pittsburgh.
Current Procedure Terminology coding and RVUs are important not only with regard to reimbursement from Medicare but also for private insurers. Although no data have been published, there is a general notion that CPT coding and RVUs are used by most private insurers as a basis for reimbursement of physician-related services.
Changes and Updates in Physician Reimbursement
As noted in Table 1 , each physician service corresponds to a CPT code. There are approximately 9000 numbers, each of which corresponds to a specific physician service. The AMA developed the CPT code and secured a copyright for its protection. (Recently the contract under which the AMA licenses use of the CPT to the government has been placed under scrutiny.) Both the development and subsequent updating of the CPT codes and the RVUs assigned to that code are conducted by the AMA partnered with the CMS.
The AMA contributes to the maintenance of the CPT coding system through the work of the AMA/Specialty RUC, which was created by the AMA in 1991. The RUC is composed of 29 members, 23 of whom are appointed by major national medical specialty societies and six of whom are representatives from the AMA CPT editorial panel, American Osteopathic Association, Health Care Professionals Advisory Committee, and Practice Expense Advisory Committee. Neurosurgery is among the 23 specialties represented on the RUC.
When a new or updated CPT code is accepted by the AMA CPT editorial panel, specialty societies are required to survey at least 30 physicians within their specialty about the physician-related work involved in the procedure. The procedure-related information gathered includes the respondent's estimate of the appropriate work relative value, the intraservice or -operative time involved, the typical number of hospital and office visits provided, and ratings of the complexity of judgment and technical skill relative to similar services. Presentations by a specialty society are very carefully examined for accuracy, data-related quality, and relativity to other physician-provided services both within and among specialties. The RUC may either adopt the specialty's recommendation or change the recommendation before it is submitted to the CMS. Final recommendations to the CMS must be adopted by a two-thirds majority of RUC members. The former's acceptance rate for RUC's recommendations has increased to more than 90% annually.[1]
The AMA also gathers data that the CMS uses to reimburse for the practice expense portion of the RVUs. Each of the direct expense totals (clinical labor, medical equipment, and medical supplies) is allocated to individual procedures based on estimates from the RUC's PEAC. Indirect costs (office expenses, administrative labor, and other expenses) are allocated to procedures based on a combination of the procedure's work RVUs and the direct practice expense estimates. If the procedure is performed by more than one specialty, a weighted average is computed based on the frequency with which each specialty performs the procedure in Medicare patients.
An important caveat to the RUC and PEAC recommendations is that the CMS must maintain budget neutrality. Federal law mandates that upward revision of certain codes or each addition of new codes must be offset by reductions in other codes. This sets the groundwork for internal conflict among the RUC members because what is good for one specialty is inherently bad for another. Recently, neurosurgery lost approximately 11% of the practice expense RVUs based on the PEAC data.
An example of how the aforementioned process can work properly can be found in coding and reimbursement for skull base surgery. In 1994, the CPT editorial panel added a new section of codes for surgery of the skull base (codes 61580-61619). Prior to 1994, there were no codes that accurately and comprehensively described the specific procedures involved in skull base surgery. The RUC surveyed its members in response to the added CPT codes with the American Academy of Otolaryngology, American Society of Plastic and Reconstructive Surgeons, and the American Association of Neurological Surgeons participating in the surveying of their respective members. These RUC members agreed on clinical vignettes, work RVUs, and typical duration of surgery, postoperative hospital stay, and number of posthospitalization visits. The entire committee ultimately accepted the recommendation of the RUC members. The RUC recommendations were passed on to Health Care Finance Administration (now the CMS) where all but four of the 28 CPT and RVU recommendations were accepted. The four recommendations rejected by the administration were open to public comment and a "refinement panel" convened by the Health Care Finance Administration and moderated by their medical officers. The refinement panel considered the four codes and voted to increase their RVUs to the RUC-recommended levels. Thus all 28 recommendations were ultimately enacted, as initially recommended by the RUC, and became effective in 1995.[1]

•   keep up to date with the latest developments in digital health
•   adopt new digital health technology
•   improve administration processes and patient care
The PIP eHealth Incentive consists of 5 eligibility requirements. For more about these, read the implementation overview documents for each requirement at the Australian Digital Health Agency website.
To be eligible to receive the eHealth Incentive, practices must:
1.   integrate Healthcare identifiers into Electronic Practice Records. To do this:
o   apply to us to obtain a Healthcare Provider Identifier-Organisation (HPI-O) for the practice and store the HPI-O in a compliant clinical software system
o   ensure that each general practitioner within the practice has their Healthcare Provider Identifier-Individual (HPI-I) stored in a compliant clinical software system, and
o   use a compliant clinical software system to access, retrieve and store verified Individual Healthcare Identifiers (IHI) for presenting patients
2.   have a standards-compliant secure messaging capability
3.   work towards recording the majority of diagnoses for active patients electronically
4.   ensure the majority of their prescriptions are sent electronically to a Prescription Exchange Service, and
5.   use the My Health Record system. To do this:
o   use compliant software for accessing the My Health Record system
o   apply to participate in the My Health Record system upon obtaining a HPI-O, and
o   upload shared health summaries to the My Health Record system for a minimum of 0.5 per cent of their Standardised Whole Patient Equivalent (SWPE) or the default SWPE, whichever is greater
The PIP eHealth Incentive encourages the meaningful use of the My Health Record by healthcare providers, including uploading of clinically relevant information to the My Health Record embedded into day to day clinical practice.

The PIP General Practitioner Aged Care Access Incentive (ACAI) aims to encourage GP's to provide increased and continuing services in Australian Government funded residential aged care facilities (RACFs).

Indigenous Health Incentive
The PIP Indigenous Health Incentive aims to support general practices and Indigenous health services to provide better health care for Aboriginal and Torres Strait Islander patients, including best practice management of chronic disease.
This incentive is a key part of the Council of Australian Governments (COAG) National Partnership Agreement on Closing the Gap: Tackling Indigenous Chronic Disease.

Procedural General Practitioner Payment
Payments are made to eligible practices in February and August each year.
Quality Prescribing Incentive (QPI)
The PIP QPI rewards participation by practices in a range of activities recognised or provided by the National Prescribing Service (NPS).
The NPS aims to assist GP's to achieve more effective, quality prescribing through a range of education, support and prescribing information. The NPS is a professional organisation, run independently of the Australian Government with broad GP representation and leadership.

Rural Loading Incentive
The rural loading recognises the difficulties of providing care, often with little professional support, in rural and remote areas. The PIP rural loading is higher for practices in more remote areas, in recognition of the added difficulties of providing medical care.

Teaching Payment
Payment to encourage general practices to provide teaching sessions to undergraduate and graduate medical students preparing to enter the Australian medical profession

Προσωπικός γιατρός μπορεί να είναι γιατρός με ειδικότητα στη γενική ιατρική, παιδίατρος για τα παιδιά κάτω των 15 ετών, γηρίατρος για τα άτομα άνω των 65 ετών, παθολόγος ή άλλος γιατρός ο οποίος, σύμφωνα με τη σχετική νομοθεσία, διαθέτει ειδική εκπαίδευση στη γενική ιατρική αναγνωρισμένη από το Ιατρικό Συμβούλιο Κύπρου ή διαθέτει πιστοποιητικό που βεβαιώνει το δικαίωμα «άσκησης δραστηριοτήτων του γιατρού γενικής ιατρικής στο πλαίσιο του εθνικού συστήματος κοινωνικής ασφάλισης στην Κύπρο ή σε άλλα κράτη-μέλη της ΕΕ».

Dr Lucia Gannon, NAGP National Council, reports on a fact-finding mission to see a new healthcare model in action in England

A delegation from Ireland, led by the NAGP, visited the UK late last month to learn about an innovative new model of primary care — the Primary Care Home.

The visit was prompted by presentations given at the Primary Care Partnership Conference in Dublin earlier this year, by both Dr Nav Chana, Chair of the National Association of Primary Care (NAPC) in the UK, and Claire Oatway, COO of the Beacon Medical Group in Plymouth.

The achievements of the Beacon Medical Group, one example of a Primary Care Home, included improved quality of care, improved staff satisfaction and significant financial savings.

While the Primary Care Home is not transferable to Ireland in its present format, it did contain interventions that could be of benefit here. The NAGP decided to explore this further, by spending two days in Plymouth visiting the Beacon Medical Group, listening to the partners, staff, and other members of the primary care team, and meeting the voluntary groups who make up this home.

The delegation included Fianna Fáil TD and Spokesperson on Primary Care and Community Health Services, John Brassil, TD; Fine Gael Seanad Spokesman on Health Senator Colm Burke; HSE National Clinical Advisor and Group Lead for Primary Care Dr David Hanlon; and HSE Policy Analyst Joan Gallagher.

NAGP members present included Dr Emmet Kerin, President; Dr Andrew Jordan, Chairman; Mr Chris Goodey, CEO; and myself, Dr Lucia Gannon.

The publication of the all-party Sláintecare Report in May outlined a 10-year vision for the future of healthcare in Ireland, and a shift away from the hospital-centric system to more primary and community-based care closer to people’s homes. The NAGP welcomed the following principles that were highlighted in the report:

• More care for patients delivered in the community;
• The implementation of an integrated care system;
• The development of general practice;
• Enhanced teams working in primary care with GP leadership;
• Better access to community diagnostics through service hubs;
• Ring-fenced transitional funding underpinned by legislation;
• An emphasis on clinical leadership, governance and State accountability.

These principles are all embodied in the Primary Care Home model of healthcare delivery.

Three aims of STPs
In the UK, it was recognised that the NHS needed to change radically to meet the healthcare needs of people in the 21st century. Sustainability and transformation plans (STPs) formed part of the NHS’s ‘Five Year Forward View’. STPs were charged with designing the future of health and social care for a defined population, and were expected to meet three aims: improved health and wellbeing; transformed quality of care delivery; and sustainable finances.

The Primary Care Home was developed by the NAPC as a means of transforming primary care delivery in a way that could meet those aims.

There are four defining characteristics of the model:

1. Provision of care to a defined registered population of between 30,000 and 50,000;
2. An integrated workforce, with a strong focus on partnerships spanning primary, secondary and social care, inclusive of patients and the voluntary sector;
3. A combined focus on personalisation of care with improvements in population health outcomes;
4. Aligned clinical and financial drivers through a unified, whole population budget with appropriate shared risks and rewards.

The Primary Care Home was formally launched in 2015. Initially, it received 67 applications from general practices around the UK. Fifteen sites were chosen as rapid test sites to pilot the project. Two years later, there are more than 191 sites involved in the project, covering 8 million patients, or 14 per cent of the population.

Creating the practice
Beacon Medical Group is a large practice, formed initially by the merger of three individual practices, which now includes four practices and covers a population of approximately 40,000 patients. It includes 30 general practitioners, 24 full partners, seven salaried general practitioners and one non-clinical partner who is the COO. It employs 140 staff, including ceptionists, nurses, clinical nurse practitioners, non-prescribing clinical pharmacists, advanced medical practitioners and admin staff.

Governance lies with the Board of partners, who meet every two weeks. Partners pooled their resources at the time of amalgamation and signed one partnership agreement.

Partners meet every four weeks. The home has a single Primary Medical Services contract and income comes from core services (60%), specialist services, services commissioned by the CCG and rental income from NHS England for surgery premises. Expenditure is decided by the partners on the basis of the needs of the local population.

General practitioners and their staff work closely with other members of the primary care and social care team. Some of these ancillary medical services have co-located to the home, in order to exchange information and formulate care plans more efficiently.

Integration of staff
The home also integrates with secondary care. Beacon Medical Centre is responsible for 10 per cent of the patients referred to the local hospital. This means that the hospital staff are interested in engaging with the medical centre on many issues, including patient referrals and discharges and access to diagnostics.

Over the two-day trip, we heard presentations from secondary care, primary care nursing, social care personnel and the voluntary sector. All outlined how the formation of the Primary Care Home had improved quality of care for patients and improved job satisfaction for staff. Among the many financial benefits accrued in the two years since its inception were:
• Reduced ED attendances (Stg£27K);
• Reduced admissions from ED (£295K);
• Reduced GP referral (330 referrals to hospital avoided);
• Reduced prescribing (£220K);
• Reduced GP waiting times (six day reduction in waiting times);
• Increased staff satisfaction (86% of staff regarded the Beacon as a good employer);
• Increased staff recruitment and retention (seven new GPs recruited in two years).

Specific initiatives also included an urgent care team to deal with same- day GP requests. This team consisted of an advanced nurse practitioner, advanced medical practitioner, clinical pharmacist, nursing team and a duty doctor, which covered all 40,000 patients from a single site.

Telephone calls were triaged by the AMP, who was dealing with up to 200 calls a day. Patients were advised and directed to the most suitable healthcare provider.

The centre had a ‘no waiting room’ policy for urgent care, meaning that rather than waiting for a GP, they would see a different member of the team if this was appropriate.

Many of these patients could be dealt with very well by a nurse or clinical pharmacist, leaving the doctor free to deal with the more complex cases.

This initiative resulted in all patients requesting urgent care being seen on the same day with no waiting time and led to significant reductions in attendances at out of hours and ED departments.

Another initiative has been a Care Home Pilot whereby a clinical pharmacist and a GP with a special interest in elder care visited six nursing and care homes once a month and provided medication reviews and physical examination.

There were a total of 1,745 patient contacts throughout the year resulting in the initiation of 302 new medications, the de-prescribing of 970 medications, leading to a saving of £83,364 (€92,750) on medication costs.

There was also a reduction in emergency admissions, reduction in OOH contacts and positive feedback from patients, care home staff and GPs.

Specialist services
Two GP partners with a special interest in musculoskeletal medicine and dermatology have also provided specialist services to the whole patient population.

This resulted in significant reductions in knee, shoulder and plastic surgery referrals. Patient and doctor satisfaction was high with this service as wait times were long in secondary care, and GPs had been providing significant monitoring for these patients.

The practice also encouraged patients to set up a patient participation group to provide feedback on services and share information on patients’ needs. This has been very successful.

The group meet every month on their own and discuss their issues. They then meet with a member of staff monthly. Because of these meetings, many new initiatives have been put in place.

The patients produce a practice newsletter, and they assist with annual flu clinics, guiding people to the appropriate person or room. They organise local health and lifestyle community events and also attend meetings concerning the integrated health and social care plan for the area, and input their concerns and wishes and actively seek funding streams where appropriate and necessary.
A further initiative has seen a liaison psychiatrist visit the practice, offering advice on mental health issues and management strategies, especially for the frequent attenders with unexplained medical symptoms.

Our impression of the Beacon Medical Group was of enthusiastic and motivated service providers who were happy in their work and interested in providing high quality care to patients in the community. Their motto was ‘right care, right person, right time’ and they worked as a highly integrated team to achieve this for individual patients, while also monitoring population health outcomes.

General practitioners felt more in control of their workload and had renewed enthusiasm for their work. Much of the work that did not need a GP was delegated to other members of the team, who were professionals with their own indemnity but felt well supported by their GP colleagues.

There was an open-door policy for all team members who could collaborate with ease throughout the day, making formal and often inefficient meetings unnecessary.

Provider autonomy
When questioned as to the success of this innovation, many people felt it worked because it allowed providers autonomy, individual skills were recognised and valued, and close connections with the voluntary sector helped care providers respond to feedback and the needs of patients quickly and effectively. Only deliverable initiatives were considered, and they were scaled to local need.

In essence, this was a bottom-up, self-organising system that was allowed to grow organically, adapt as necessary, celebrate successes and learn from failures.

It was based on collaboration throughout the whole primary care system and staff felt that they were empowered drivers of change.

The Primary Care Home model has the potential to bring higher patient satisfaction along with a safer, more cost-effective health service.

Many of the interventions contained in this model could be easily adapted for Ireland.

ΟΙΚΟΓΕΝΕΙΑΚΟΣ ΙΑΤΡΟΣ
ΕΙΣΑΓΩΓΗ
Ο ρόλος του Οικογενειακού Ιατρού στο σύστημα υγείας θεσμοθετείται από νόμους του
Κράτους. Εφόσον το Κράτος εισαγάγει τον θεσμό του Οικογενειακού Ιατρού, καλούνται
οι αντίστοιχοι ιατροί να εφαρμόσουν τον Νόμο και να λειτουργήσουν ως Οικογενειακοί
Ιατροί.
Η λειτουργία του Οικογενειακού Ιατρού συνίσταται στην καθολική κάλυψη των
αναγκών υγείας σε επίπεδο Πρωτοβάθμιας Φροντίδας Υγείας.
Επί προηγούμενων κυβερνήσεων έγινε προσπάθεια δημιουργίας δικτύου
πρωτοβάθμιας φροντίδας υγείας, ως βασικός μοχλός εκσυγχρονισμού του συστήματος
υγείας και μείωσης των δαπανών υγείας.
Το τελευταίο νομοθετικό πλαίσιο που ορίζει το πλαίσιο εφαρμογής και λειτουργίας του
Οικογενειακού Ιατρού είναι ο Ν. 4486/2017.
Βασικό πρόβλημα στην εφαρμογή του συγκεκριμένου προγράμματος υπήρξε η άρνηση
των ιατρών να ενταχθούν στο νέο σύστημα λόγω κυρίως χαμηλών αμοιβών, αλλά και
απωθητικών εργασιακών σχέσεων.
ΠΡΟΤΑΣΕΙΣ
ΟΙΚΟΓΕΝΕΙΑΚΟΣ ΙΑΤΡΟΣ
Ο Οικογενειακός Ιατρός είναι ο ιατρός που καλύπτει καθολικά τις ανάγκες υγείας σε
επίπεδο Π.Φ.Υ. και χρειάζονται περίπου 7.000 Οικογενειακοί ιατροί για την
ολοκληρωτική κάλυψη του πληθυσμού της χώρας. Το ρόλο του Οικογενειακού Ιατρού
δύνανται να αναλάβουν Γενικοί Ιατροί ή Παθολόγοι για ενήλικες και Παιδίατροι για
παιδιά. Απαιτείται πρόγραμμα «on the job training» για την κατάλληλη επιμόρφωση
των ιατρών.
Ειδικά σε ό,τι αφορά τη φροντίδα των παιδιών μέχρι 16 ετών, υπεύθυνος οικογενειακός
ιατρός θα είναι ο εξειδικευμένος Παιδίατρος. Όπου και όποτε υπάρχει αδυναμία να
απευθυνθούν οι γονείς σε Παιδίατρο, χρέη Οικογενειακού Ιατρού θα εκτελεί ο θεράπων
ιατρός και των υπολοίπων μελών της οικογένειας.
ΕΠΙΛΟΓΗ ΟΙΚΟΓ. ΙΑΤΡΟΥ
Η ελεύθερη επιλογή οικογενειακού ιατρού θα πρέπει να διασφαλίζεται, μέσα από ένα
δίκτυο στο οποίο θα εντάσσονται όσοι το επιθυμούν, με μακροχρόνια πρόβλεψη
εναρμόνισης αναλογίας οικογενειακών ιατρών και πληθυσμού.
Η διαδικασία σύνδεσης των ιατρών θα πρέπει να ακολουθεί τους εξής άξονες:

α) Οριοθέτηση ενιαίου ηλεκτρονικού αλγορίθμου διαχείρισης ασθενών και
πρωτοκόλλων αντιμετώπισης που θα εφαρμόζεται από όλους τους οικογενειακούς
ιατρούς. Θεσμοθέτηση ηλεκτρονικού φακέλου ασθενών, η τήρηση του οποίου θα είναι
ευθύνη του οικογενειακού ιατρού και η ενημέρωσή του υποχρέωση και των ειδικών
ιατρών, που τυχόν εξετάζουν τον ασθενή.
β) Οι οικογενειακοί ιατροί δύνανται να είναι του δημοσίου, με σύμβαση,
είτε πιστοποιημένοι με προκαθορισμένη αμοιβή, ελεύθεροι επαγγελματίες.
Έτσι θα επιτευχθεί:
• Ίση μεταχείριση των πολιτών τόσο στο δημόσιο όσο και στον ιδιωτικό τομέα .
• Πραγματικά ελεύθερη επιλογή οικογενειακού ιατρού και όχι κατευθυνόμενη
και εξαναγκαστική.
Επιπλέον, θα πρέπει να δημιουργηθεί ένα παράλληλο δίκτυο ιατρών λοιπών
ειδικοτήτων που θα συνεργάζεται με το δίκτυο οικογενειακών ιατρών.
ΚΑΤΑΛΟΓΟΣ ΑΣΘΕΝΩΝ
Ο κατάλογος των πολιτών κάθε Οικογενειακού Ιατρού συγκροτείται από τους πολίτες
που τον επέλεξαν.
Συγκεκριμένα, στην αρχή κάθε έτους, από 1 – 15 Ιανουαρίου, αναρτάται, μερίμνη των
Περιφερειακών Διοικήσεων του Υπουργείου Υγείας, κατάλογος των διαθέσιμων
Οικογενειακών Ιατρών κάθε περιοχής και οι πολίτες επιλέγουν έναν από αυτούς. Ο
κατάλογος (λίστα) των ασθενών κλείνει όταν ο ιατρός συμπληρώσει 1500 πολίτες.
Οι κατάλογοι (λίστες) των Οικογενειακών Ιατρών λόγω ιδιαιτέρων χαρακτηριστικών,
όπως η πληθυσμιακή κατανομή της χώρας αλλά και ειδικών κατηγοριών, όπως η ηλικία,
το φύλο, τα χρόνια νοσήματα, ο βαθμός νοσηρότητας κλπ., διαβαθμίζονται σε
κατηγορίες, οι οποίες συνιστούν διαφοροποιημένα οικονομικά μεγέθη της αμοιβής των
ιατρών.
Ο προβλεπόμενος μέγιστος πληθυσμός ευθύνης των 2.250 ασθενών για κάθε ιατρό,
ήταν πέρα από κάθε διεθνή επιστημονική πρακτική και εγκυμονεί κινδύνους σοβαρών
ιατρικών λαθών.
Συνεπώς, προτείνεται ως μέγιστος αριθμός ασφαλισμένων οι 1.500 ανά ιατρό, βάσει
των μέσων όρων της Ευρωπαϊκής Ένωσης.

ΠΑΡΑΠΟΜΠΗ ΣΕ ΕΞΩΝΟΣΟΚΟΜΕΙΑΚΗ/ΝΟΣΟΚΟΜΕΙΑΚΗ ΦΡΟΝΤΙΔΑ –
ΝΟΣΟΚΟΜΕΙΑΚΗ ΠΕΡΙΘΑΛΨΗ
Όταν ο πολίτης έχει ανάγκη εξειδικευμένης εξωνοσονοκομειακής ή νοσοκομειακής
φροντίδας, ο Οικογενειακός Ιατρός τον παραπέμπει σε δευτεροβάθμια και τριτοβάθμια
Νοσηλευτικά Ιδρύματα του ΕΣΥ και του ιδιωτικού τομέα. Δυνατότητα παραπομπής έχει
αυτοδίκαια και κάθε ειδικός ιατρός, ανεξαρτήτως θέσεως και σχέσης εργασίας για
περιπτώσεις σχετικές με το επιστημονικό του πεδίο.
Η πρόσβαση στις δευτεροβάθμιες και τριτοβάθμιες δομές απαιτεί παραπομπή και
ενημέρωση του φακέλου των ασθενών (εκτός ατυχημάτων και επειγόντων). Όταν η
παραπομπή γίνεται από μη οικογενειακό ιατρό, ο παραπέμπων οφείλει να ενημερώνει
τον οικογενειακό ιατρό που τηρεί το φάκελο του ασθενούς.
Επίσης για να διασφαλισθεί απρόσκοπτα η 24ώρη παροχή υπηρεσιών
προνοσοκομειακής φροντίδας και να αποσυμφορηθούν τα Νοσοκομεία, θα πρέπει να
υπάρχουν Κέντρα Υγείας Αστικού Τύπου 24ωρης λειτουργίας με συμμετοχή και ιδιωτών
συμβεβλημένων ιατρών στις εφημερίες (όπου δεν επαρκεί ο αριθμός των
υπηρετούντων ιατρών του δημοσίου) ή και εφημερίες πόλεως των ιδιωτών
οικογενειακών ιατρών, με πρόγραμμα που θα εκδίδει ο κάθε Ιατρικός Σύλλογος.
Ο αριθμός των νοσοκομειακών κλινών οξείας νοσηλείας το 2015 ήταν 360 ανά 100.000
κατοίκους, ο οποίος δεν είναι μόνο κάτω από τον μέσο όρο της ΕΕ που είναι 408, αλλά
καταδεικνύει επίσης μεγαλύτερη δαπάνη στη Νοσοκομειακή περίθαλψη από τις
συνολικές δαπάνες υγείας της χώρας.
Ο μέσος όρος των δαπανών υγείας στην Ε.Ε. για τα νοσοκομεία είναι 28% και στην
Ελλάδα 40 %.
Με βάση τα παραπάνω, προτείνονται τα εξής:
• Άμεση κάλυψη των κενών θέσεων ειδικών και ειδικευομένων, με παροχή
κινήτρων (μισθολογικά, μετεκπαίδευση, εξέλιξη κλπ)
• Ενίσχυση και επέκταση της λειτουργίας των ΤΕΠ,
• Ορθολογική κατανομή των πόρων του ΕΟΠΥΥ, αναλογικά και στις 3
βαθμίδες του συστήματος με έμφαση στην ΠΦΥ.
ΠΦΥ – ΤΟΜΥ
Αρχικά προβλεπόταν η δημιουργία 239 ΤΟΜΥ και η πρόσληψη 1.195 ιατρών. Όμως μέχρι
στιγμής δεν είναι πάνω από 550 οι ιατροί που έχουν προσληφθεί, ενώ οι ΤΟΜΥ σε όλη
τη χώρα είναι περίπου 100.
Από την 1η Αυγούστου του 2018 ακυρώθηκαν αιφνιδιαστικά και σχεδόν εκβιαστικά
περίπου 1600 συμβάσεις Παθολόγων με τον ΕΟΠΥΥ με το σύστημα των 200 επισκέψεων

σε σύνολο περίπου 3.200 Παθολογικών Ιατρείων. Να σημειώσουμε ότι οι 200
επισκέψεις που αποζημίωνε ο ΕΟΠΥΥ εξαντλούνταν τις πρώτες 7 – 10 ημέρες κάθε μήνα.
Προτείνονται:
• Κάλυψη κενών θέσεων σε ΤΟΜΥ/Κέντρα Υγείας μέχρι τέλος του 2019.
• Παροχή κινήτρων (μισθολογικό, καθεστώς συμβάσεων π.χ. πλήρους και
αποκλειστικής, πλήρους αλλά και μερικής απασχόλησης).
ΗΛΕΚΤΡΟΝΙΚΟΣ ΦΑΚΕΛΟΣ ΑΣΘΕΝΟΥΣ
Ο κάθε πολίτης διαθέτει φάκελο ιστορικού, για την διαχείριση του οποίου είναι
υπεύθυνος ο Οικογενειακός Ιατρός του.
Σ’ αυτόν τον φάκελο συμπεριλαμβάνονται και τα σημειώματα του ιατρού του, που
αναφέρονται σε όλες τις τυχούσες παραπομπές, όπως και οι προβλεπόμενες από τον
Νόμο αναφορές των Ειδικών Ιατρών της Δευτεροβάθμιας Περίθαλψης προς τον
Οικογενειακό Ιατρό, όπου περιγράφεται η εκάστοτε επέμβαση ή όποια άλλη θεραπεία
χορηγήθηκε στον παραπεμφθέντα ασθενή, καθώς και αντίστοιχα σημειώματα άλλων
επαγγελματιών υγείας.
Συνεπώς προτείνεται η θεσμοθέτηση ηλεκτρονικού φακέλου ασθενών, η τήρηση του
οποίου θα είναι ευθύνη του οικογενειακού ιατρού και η ενημέρωσή του υποχρέωση
και των ειδικών ιατρών που τυχόν εξετάζουν τον ασθενή.
ΣΧΕΣΕΙΣ ΕΡΓΑΣΙΑΣ - ΑΠΟΖΗΜΙΩΣΕΙΣ - ΑΜΟΙΒΕΣ/ΜΙΣΘΟΛΟΓΙΟ
Συλλογικές συμβάσεις μέσω Π.Ι.Σ., με συμμετοχή των Επαγγελματικών Ενώσεων δια
των Ιατρικών Συλλόγων.
Οι εργασιακές σχέσεις όλων των Οικογενειακών Ιατρών της Επικράτειας θα είναι
ετήσια συμβόλαια παροχής πλήρους και όχι αποκλειστικής απασχόλησης. Κατά το
ωράριο απασχόλησής του ο Οικογενειακός Ιατρός απαγορεύεται να εξετάζει ή και να
απασχολείται με οποιονδήποτε τρόπο από ασθενείς εκτός της λίστας του.
Στα συμβόλαια αυτά πρέπει να προβλέπεται κανονική και εκπαιδευτική άδεια.
Αξιοπρεπείς συμβάσεις τόσο για τους ιατρούς των δημόσιων δομών, όσο και για τους
ελευθεροεπαγγελματίες συμβεβλημένους. Ίσα δικαιώματα με τους υπαλλήλους άλλων
κατηγοριών του ευρύτερου δημόσιου τομέα (ασφάλιση, άδειες, επιδόματα).
Οι οικογενειακοί ιατροί να είναι είτε του δημοσίου, είτε με σύμβαση, είτε
πιστοποιημένοι, με προκαθορισμένη αμοιβή. Έμπρακτος σεβασμός στο έργο των
συμβαλλόμενων ιατρών με αξιοπρεπή και δίκαιη αμοιβή.

Στους οικογενειακούς ιατρούς με συμβάσεις εξαρτημένης εργασίας να προβλέπεται η
πληρωμή άδειας 30 εργάσιμων ημερών, καθώς και ασφαλιστικές εισφορές εργοδότου
από τον ΕΟΠΥΥ, όπως προβλέπει ο νόμος.
Για απλή συνταγογράφηση αμοιβη 10 ευρω.
Για επικέψεις στο σπίτι θα πρέπει να υπάρχει η δυνατότητα συμμετοχής του ασθενούς
η της ασφαλιστικής του εταιρείας, με προκαθορισμένη κλειστή αμοιβή γιατρού.
Επιπλέον:
Θεσμοθέτηση πάγιων ετησίων αποζημιώσεων για:
• Χρήση του χώρου
• Τήρηση αρχείων (δεδομένου ότι θα ζητηθούν πιστοποιήσεις για τήρηση του στο
άμεσο μέλλον)
• Πιστοποιήσεις που χρειάζονται τα ιατρεία
• Απασχόληση γραμματειακής υποστήριξης
• Να υπάρχει η δυνατότητα συμμετοχής του ίδιου του ασθενούς η της ασφαλιστικής
εταιρείας του, με καθορισμένη κλειστή αμοιβή γιατρού, επι πλέον από την αμοιβή
του ΕΟΠΥΥ
• Μηνιαία αποζημίωση για
a. χρήση του χώρου
b. απασχόληση γραμματειακής υποστήριξης
Ο σχεδιασμός θα πρέπει να βασίζεται στις ανάγκες του πληθυσμού και εν συνεχεία να
προσαρμόζονται οι οικονομικοί πόροι σε αυτό. Θεωρούμε την αποκλειστική
χρηματοδότηση του Οικογενειακού Ιατρού από τον ΕΟΠΥΥ ανεφάρμοστη και ανεδαφική
διότι για την Π.Φ.Υ. είναι προϋπολογισμένα 105 εκατομμύρια για τις επισκέψεις των
Ιατρών όλων των ειδικοτήτων. Οι ιδιωτικές δαπάνες για την υγεία, κυρίως με τη μορφή
άμεσων πληρωμών από τους ασθενείς, ήταν ανέκαθεν σημαντικό χαρακτηριστικό του
ελληνικού συστήματος υγείας και θεωρούμε πως με τους συρρικνωμένους
προϋπολογισμούς υπέρ υγείας και ειδικά υπέρ Πρωτοβάθμιας και οικογενειακού
ιατρού, δεν μπορεί να αλλάξει ριζικά το σύστημα υγείας στη χώρα.
Οι ιατροί χωρίς σύμβαση να φέρουν πιστοποίηση Οικογενειακού ιατρού, προκειμένου
να διευρυνθεί το δίκτυο ιατρών και η πρόσβαση των πολιτών στο σύστημα.
Γενικά, η αμοιβή του ιατρού συνιστά ένα ιδιαίτερα σοβαρό ζήτημα. Θεωρούμε πως η
αξία της ιατρικής πράξης θα πρέπει να αποτιμάται από το ποσό κοστολόγησης του
ιατρικού λάθους.
Επειδή όμως αυτό είναι ουσιαστικά ανέφικτο προτείνονται τα εξής:

Βελτίωση αμοιβών ιατρών ώστε να είναι ελκυστικές.
1) Αμοιβές Δημόσιων οικογενειακών ιατρών: Συναφείς με το μισθολόγιο και το ωράριο
του Ε.Σ.Υ.
2) Αμοιβές συμβεβλημένων: Εφαρμογή της κατά κεφαλήν αποζημίωσης (capitation) με
ετήσια εισφορά 50 ευρώ ανά εγγεγραμμένο ασθενή. Το ποσό θα μπορούσε να
καλυφθεί πλήρως ή εν μέρει από τον ΕΟΠΥΥ.
3) Αμοιβές πιστοποιημένων με Ελεύθερη αμοιβή: Δυνατότητα να εκπίπτει ποσό
αντίστοιχο με την ετήσια κατά κεφαλή αποζημίωση από τον ΕΟΠΥΥ από τον τελικό
ετήσιο φόρο.
Κατά συνέπεια, οι αμοιβές των Οικογενειακών Ιατρών αποδίδονται με ένα σύστημα
κατά́ κεφαλήν (per capita) για συγκεκριμένες ποσοτικές και ποιοτικές υπηρεσίες, οι
οποίες θα περιλαμβάνονται στους όρους του συμβολαίου. Υπηρεσίες εκτός σύμβασης
(όπως πχ οι κατ’ οίκον επισκέψεις κλπ.) θα αποζημιώνονται ξεχωριστά. Σε ειδικές
περιπτώσεις (πχ επιδημίες κλπ.) θα μπορεί ο αγοραστής υπηρεσιών υγείας να
αποζημιώνει τον Οικογενειακό Ιατρό, κατά πράξη και περίπτωση (fee for service) για
ειδικές πρόσθετες υπηρεσίες. Η ΕΛΕΓΕΙΑ κρίνει ότι αμοιβή κατά κεφαλήν ύψους 40
ευρώ κατ’ έτος είναι ικανοποιητική και έχοντας υπ’ όψιν την προτροπή του Π.Ο.Υ.
«απαιτείται minimum συμμετοχή των πολιτών στις δαπάνες υγείας», προτείνει την
καταβολή εκ μέρους των πολιτών «Παραβόλου Επιλογής» ύψους από 10 ευρώ (25%)
κατ’ έτος, με βάση τα εισοδηματικά κριτήρια, την ώρα που θα επιλέγουν τον
Οικογενειακό τους Ιατρό. Προτείνεται οι άνεργοι πολίτες να απαλλάσσονται από το
παράβολο επιλογής.
Θα υπάρξει μεταβατικό διάστημα, κατά το οποίο οι Οικογενειακοί γιατροί των ΚΥ και ΠΙ
θα ερωτηθούν αν επιθυμούν ή όχι να συμμετάσχουν στο νέο σύστημα. Αν όχι,
διατηρούν τις εργασιακές τους σχέσεις και τις αμοιβές τους. Αν δέχονται, προχωρούν σε
υπογραφή συμβολαίου με αμοιβή «per capita» χωρίς αποκλειστική απασχόληση κλπ.
Ορθολογική κατανομή πόρων. Δεν είναι δυνατόν ο Ε.Ο.Π.Υ.Υ. να έχει διαχειριστικά
έξοδα 176 εκατομμύρια ετησίως και για την παροχή υγείας στους πολίτες να
διατίθενται μόνο 90 εκατομμύρια. Αυτό σημαίνει ότι η Πολιτεία αντιμετωπίζει το ίδιο
εξευτελιστικά ιατρούς και πολίτες. Επίσης οι πόροι πρέπει να κατανέμονται ισόρροπα
προς τους ασθενείς, ανεξαρτήτως αν αυτοί επιθυμούν να εξυπηρετούνται από δημόσιες
δομές ή συμβεβλημένους ιατρούς. Στην συγκεκριμένη συγκυρία και καθώς το πείραμα
των ΤΟΜΥ δε φαίνεται να επιτυγχάνει, ζητούμε οι πόροι του ΕΣΠΑ να ανακατανεμηθούν
προς τον ΕΟΠΥΥ.
ΩΡΕΣ ΕΡΓΑΣΙΑΣ
Οι ώρες εργασίας του Οικογενειακού Ιατρού στο σύστημα θα πρέπει να είναι
αντίστοιχες του μεγέθους της λίστας ασθενών του, φθάνοντας για Ο/Ι με

συμπληρωμένη τη λίστα τους, τις 7 ώρες καθημερινά ή 35 ώρες εβδομαδιαίως εκτός
Σαββάτου και Κυριακής και αργιών. Για τις υπόλοιπες ώρες και ημέρες θα πρέπει να
υπάρξει μηνιαίο πρόγραμμα ετοιμότητας (on call) στο οποίο συμμετέχουν υποχρεωτικά
όλοι οι Ο/Ι και το οποίο ρυθμίζεται και δημοσιοποιείται μηνιαίως από τις αντίστοιχες
Υγειονομικές Περιφέρειες (ΥΠΕ)
Οι συμβεβλημένοι ιατροί με τον ΕΟΠΥΥ να παρέχουν σε όλο το εύρος του ωραρίου
τους τις υπηρεσίες τους με τους ίδιους όρους για τους ασφαλισμένους που τους έχουν
επιλέξουν. Είμαστε αντίθετοι με το σύστημα: πρωί δωρεάν παροχή υπηρεσιών των
συμβεβλημένων και το απόγευμα ελεύθερο επάγγελμα καθώς ενδεχομένως να συνιστά
αθέμιτο ανταγωνισμό και ίσως να δημιουργούνται συνθήκες παραοικονομίας.
ΕΠΑΓΓΕΛΜΑΤΙΚΗ ΚΑΤΟΧΥΡΩΣΗ
Ενίσχυση και αυστηροποίηση του νομοθετικού πλαισίου, με σκοπό την προάσπιση
και την διαφύλαξη της υγείας του πληθυσμού και ιδιαίτερα των ευαίσθητων ομάδων.
Αυστηρότερες δικλείδες, μέσω και της ΗΔΙΚΑ, κατά της αντιποίησης του ιατρικού
επαγγέλματος και της ειδικότητας της Παιδιατρικής. Χορήγηση ΑΔΥΜ, βεβαιώσεων
καλής υγείας, συνταγογράφηση σε ηλικίες κάτω των 16 ετών μόνο από
Παιδιάτρους. Εντατικοποίηση ελέγχων έναντι «επαγγελμάτων» και μη
νόμιμων και θεσμικά κατοχυρωμένων εξειδικεύσεων (σύμβουλοι θηλασμού,
σύμβουλοι μητρότητας κλπ).
Καθολική απαγόρευση χορήγησης φαρμάκων χωρίς ιατρική συνταγή. Ελεύθερη
χορήγηση μόνο των ΜΗΣΥΦΑ.
ΠΟΙΟΤΗΤΑ ΥΠΗΡΕΣΙΩΝ- ΠΛΑΙΣΙΟ ΛΕΙΤΟΥΡΓΙΑΣ
Η οργάνωση των ραντεβού να είναι αποκλειστική ευθύνη του ιατρού
Μείωση της γραφειοκρατίας προκειμένου να αυξηθεί ο χρόνος εξέτασης του ασθενούς.
Οι μέχρι τώρα παρεμβάσεις της ΗΔΙΚΑ στην ηλεκτρονική συνταγογράφηση και τον
ιατρικό φάκελο θεωρούμε, ότι βοηθούν προς το σκοπό αυτό, αλλά αυξάνουν την
γραφειοκρατία και κρατούν τον ιατρό όμηρο γραφειοκρατικών διαδικασιών που
στερούν ενδεχομένως υπηρεσίες προς τον ασθενή.
Άνω των 500 εγγεγραμμένων θα πρέπει ο ιατρός με οποιαδήποτε σχέση εργασίας να
διαθέτει γραμματειακή υποστήριξη, επιδοτούμενη από προγράμματα του ΟΑΕΔ ή τον
ΕΟΠΥΥ.
Να δημιουργηθούν προγράμματα επιδότησης νοσηλευτών για ιατρεία πρωτοβάθμιας.
Η Ελλάδα είναι η χώρα με τον μικρότερο αριθμό νοσηλευτών σε σχέση με τον μέσο όρο

της Ε.Ε. με σχεδόν καθολική απασχόληση στη δευτεροβάθμια περίθαλψη. Παράλληλα
έχει και τον μεγαλύτερο αριθμό άνεργων νοσηλευτών. Ο εξορθολογισμός αυτών των
δεδομένων σίγουρα θα επιτευχθεί με την ενασχόληση νοσηλευτών στα ιατρεία Π.Φ.Υ.
ΕΠΙΛΟΓΟΣ
Σε ένα κόσμο που η ιατρική γνώση εξειδικεύεται συνεχώς, οι πρωτοβάθμιοι και
εξειδικευμένοι ιατροί θα πρέπει να είναι σε άμεση συνεργασία για το καλό των
πολιτών.
Στην Ελλάδα του σήμερα όπου υπάρχει πληθώρα ιατρών ειδικοτήτων, αυτό είναι
εφικτό και αποτελεί παγκόσμιο προνόμιο για τους Έλληνες πολίτες, εάν υπάρξει η
έμπνευση και η πολιτική βούληση.

Ορισμένα σχόλια επι της draft πρότασης του ΠΙΣ:
-   Ο αποκλειμός των ΓΙ από την δυνατότητα να παρακαλουθεί παιδιά κάτω των 16 έτων είναι απλώς ΠΑΡΑΝΟΜΟΣ.
-   Ο ΠΙΣ πρέπει να καταλήξει σε ένα κείμενο – συμβόλαιο ΟΙ όπου θα καθοριζει το πλαίσιο λειτουργίας του θεσμού .
-   Οσοι πρωτοβάθμιοι ιατροί (ιδιωτικού και δημοσίου τομέα )επιθυμούν να υιοθετήσουν τον ρόλο του ΟΙ εγγράφονται  στην λίστα ΟΙ της οικίας ΥΠΕ - primary care provider (PCP) directory.
-   O ΕΟΠΥΥ δεν κάνει καμία σύμβαση με κανένα ΟΙ.
-   Ο ΕΟΠΥΥ εκδίδει e- voucher  ανα ασφαλισμένο.
-   Ο ασφαλισμενός εαν επιλέξει  ΟΙ  του δημοσίου τότε το voucher ενισχύει το budget της μονάδας υγείας στην οποία υπάγεται.
-   Εαν επιλέξει ιδιώτη ΟΙ προσκομίζει το voucher και πληρώνει απο την τσέπη του gap fee. H διαφορά αυτή θα είναι μερικώς refundable απο την ιδιωτική του ασφάλεια και το υπόλοιπο ποσό μερικώς refundable απο την εφορία.
-   Ο ΠΙΣ δεν θα ρυθμίζει την αμοιβή συλλογικά ,παρά μόνο τους όρους του συμβολαίου.
-   Η διαστρωμάτωση της λίστας ανάλογα με την γραμματειακή υποστήριξη είναι ανεδαφική και μη εφαρμόσιμη. Διοτι απλούστατα εαν έχεις γραμματειακη υποστηριξη και γεμάτη λίστα, εαν σου φύγει η γραμματέας δεν ειναι ουτε σωστό αλλά και ούτε εφικτό να διώξεις τους ασθενείς σου.

Κάποια δικά μου σχόλια:

α) Αυτός που έγραψε το "Όπου και όποτε υπάρχει αδυναμία να απευθυνθούν οι γονείς σε Παιδίατρο, χρέη Οικογενειακού Ιατρού θα εκτελεί ο θεράπων ιατρός και των υπολοίπων μελών της οικογένειας" θα πρέπει να μας εξηγήσει ποιος πληρώνεται το "χρέη Οικογενειακού Ιατρού". Τα πληρώνει ο παιδίατρος που αδυνατούσε ή τα πληρώνει η οικογένεια του παιδιού; Και πόσο πρέπει να αδυνατεί ο παιδίατρος και ποιος το κρίνει; Ή ξεχωρίζεις τελείως ότι κάτω από τα 16 μόνο στον παιδίατρο ή επιτρέπεις στο γονιό να διαλέξει αν ο ίδιος το θέλει ως οικογενειακό ιατρό ιατρό με ειδικότητα γενικής ιατρικής.

β) Θα πρέπει να μην απασχολεί τον πολίτη που βρίσκεται και πως λειτουργεί ο οικογενειακός ιατρός. (σε ιδιωτική ή κρατική μονάδα). Άρα χρειάζονται ενιαίοι κανόνες για κάθε γιατρό που μπαίνει στο θεσμό: Από τις ώρες εργασίας/εβδομάδα έως το πως αναπληρώνεται στην απουσία του, από το τι στόχους τίθενται έως τι απαιτήσεις σε αποτελέσματα υπάρχουν, από τις παραπομπές έως τις παροχές. Ο οικογενειακός ιατρός ενημερώνει τον φάκελο υγείας και παραπέμπει με ηλεκτρονικό παραπεμπτικό, για το οποίο έρχεται ηλεκτρονική απάντηση  από τον ειδικό και παραμένει μέσα στο φάκελο του ασθενή.

γ) Θα πρέπει να υπάρξει μέριμνα για πολίτες που για ένα χρονικό διάστημα άνω του μηνός βρίσκονται σε άλλη κατοικία τους. πχ υπάρχουν εργαζόμενοι σε τουριστικές περιοχές το καλοκαίρι και σε άλλη το χειμώνα. Δεν μπορεί ο οικογενειακός ιατρός να πηγαίνει μόνο με το έτος. Θα πρέπει να έχει περιθώριο να επιλέξει ένας πολίτης τον οικογενειακό γιατρό της αρεσκείας του αλλά θα πρέπει αιτιολογημένα να μπορεί και ο γιατρός να αποκλείσει ένα πολίτη από να τον επιλέξει ως οικογενειακό ιατρό.

δ) Ο οικογενειακός ιατρός θα πρέπει να είναι μια υπηρεσία για τον πολίτη ανεξάρτητα ασφαλιστικής κάλυψης ή όχι. Άρα εδώ ο ΕΟΠΥΥ δεν έχει καμιά δουλειά. Είναι δικαίωμα που προβλέπεται από το σύνταγμα και πρέπει να προέρχεται από την φορολογία. Είναι στοιχείο της δημόσιας υγείας (public health) και η δαπάνη του είναι μικρή συγκριτικά με τις λοιπές δαπάνες υγείας.

ε) Οικονομικά είναι ασύμφορο και άσκοπο να φτιάξεις επιπλέον κέντρα υγείας για να κάνουν 24ωρη κάλυψη εκτός κι αν καταργήσεις τα ΤΕΠ. θα μπορούσαν όποιοι οικογενειακοί ιατροί θέλουν συμπλήρωμα στο μισθό τους να κάνουν εφημερίες στα ΤΕΠ. Τα ΤΕΠ όμως πρέπει να αποκτήσουν μεγαλύτερη λειτουργική διασύνδεση με την ΠΦΥ από ότι με το ίδιο το νοσοκομείο. Επίσης, θα πρέπει να επιδοτηθεί η συνεργασία οικογενειακών ιατρών για εφημερίες στο χώρο τους μέσα από την δημιουργία της μορφής της κοινωφελούς επιχείρησης.

στ) Υπάρχουν πολλά σοβαρά θέματα στις συμβάσεις για τους οικογενειακούς ιατρούς: Μια προσφιλής κερκόπορτα του συστήματος επί ΙΚΑ, για να μεταθέσει το κόστος στον ασθενή, ήταν ο οικογενειακός ιατρός βλέπει τους πολίτες της λίστας του σε συγκεκριμένο ωράριο και μετά μπορεί να τους ζητήσει χρήματα αν έρθουν εκτός ωραρίου. Αυτό όμως καταστρατηγεί την ελεύθερη πρόσβαση στον οικογενειακό ιατρό. Αντίθετα, το σωστό σε μια σύμβαση είναι ότι από ασθενή της λίστα σου, δεν μπορείς να πάρεις χρήματα για κανένα λόγο. Το επόμενο πρόβλημα είναι πως αντιμετωπίζεις την υπέρμετρη χρησιμοποίηση των υπηρεσιών υγείας από τον πολίτη. Αυτό στο εξωτερικό αντιμετωπίζεται με τα ραντεβού και με την διαλογή από την νοσηλεύτρια. Εννοείται ότι δεν μπορεί να χρησιμοποιηθεί τίποτα διαφορετικό από το capitation, ότι πρέπει να υπάρχει ISO ιατρεία και ότι δεν θα μετράμε γραμματείς και αριθμό πολιτών στην λίστα.

Γιατί δεν αφήνουμε την ελευθερη οικονομία με παράλληλη κρατική μέριμνα να μη φτάσει η κατάσταση στα άκρα και να υπάρχει έλεγχος ποιότητας;
