Τις εξετάσεις προσυμπτωματικού ελέγχου γενικά τις δίνω κατά περίπτωση, ανάλογα με το ιστορικό.
Ο γενικός έλεγχος για άτομα με ελεύθερο ιστορικό που χρησιμοποιώ είναι:
γενική αίματος, ουρία, κρεατινίνη, σάκχαρο, ολική χοληστερόλη, τριγλυκερίδια.
Αν κάποιος είναι >50 συστήνω κολονοσκόπηση, αν θέλει να κάνει και PSA του το γράφω. Αντίστοιχα για μαστογραφίες.
Κι αν με πρήζει επί ένα τέταρτο παρ'ό,τι του εξηγώ ότι κάποιες εξετάσεις δεν βοηθούν ή αν διαπιστώνω ότι είναι περιττό να εξηγήσω, μπορεί να ενδώσω και να γράψω διάφορα, αν έστω με δημιουργική λογική μπορούν να δικαιολογηθούν, προκειμένου να πάω στον επόμενο...

    Προσωπικά στις προληπτικες εξετασεις μερικες φορες βαζω και την γλυκοζυλιωμενη αιμοσφαιρινη (HbA1c), κυρίως  αν ο ασθενής είναι παχύσαρκος. Μπορώ ετσι  να εντοπίσω είτε σακχαρώδη διαβήτη (όταν η τιμή είναι > 6,4%), είτε, συχνότερα, προδιαβήτη (όταν η τιμη είναι > 5,7 και < 6,4). Οι τιμες πρωϊνού σακχάρου νηστείας συχνά είναι κατώτερες από τις θεωρητικά αντίστοιχες των επιπεδων  γλυκοζυλιωμενης  αιμοσφαιρινης (δηλαδή να είναι το σάκχαρο νηστείας >126 mg όταν η HbA1c  είναι > 6,4%  και 110-126 mg (διαταραχή γλυκοζης νηστειας) όταν η HbA1c  είναι > 5,7% και <6,4%). Πρόσφατα ειχα ασθενή με  σακχαρο νηστείας  98 mg  και HbA1c  6,2% , δηλαδή  προδιαβητικό, που με αυτόν τον τρόπο θα ωφεληθεί από την έγκαιρη διάγνωση, εάν αλλάξει διατροφικες συνηθειες, ασκηθεί και χάσει βάρος βέβαια.
    Επίσης κατά κανόνα  μετράω επίπεδα βιταμίνης D  (για ακρίβεια την 25-OH-βιταμινη D), εκτος από την περίοδο Μαΐου – Οκτωβρίου. Η  επάρκεια  βιταμίνης  D αποτελεί ασπίδα για μια πλειάδα νοσημάτων (όπως έχει  δείξει η σύγχρονη έρευνα και έχουμε  συζητησει  στο φόρουμ Δεν είναι ορατοί οι σύνδεσμοι (links). Εγγραφή ή Είσοδος)  και θεωρώ ότι ο γενικος γιατρός, ως κατ’ εξοχήν γιατρός πρόληψης, πρεπει να εντάξει τη μέτρηση της στην φαρέτρα των προληπτικών εξετάσεων για τους ασθενείς του.

Ο Γιώργος Νότας είναι Λέκτορας Εργαστηριακής Ενδοκρινολογίας στο Πανεπιστήμιο της Κρήτης. Στο 2ο Πανελλήνιο Σεμινάριο Επείγουσας Ιατρικής, που έγινε στην Καβάλα τον Απρίλιο του 2014, έδωσε μια εξαιρετική ομιλία όπου περιέγραφε τι κοστίζουν οι σπατάλες των εργαστηριακών εξετάσεων στα ΤΕΠ.

Παρακάτω παραθέτω την απάντησή του, αφού πήρα πρώτα την έγκρισή του να την δημοσιεύσω:


Καλημέρα,
Ενδιαφέρουσα η προσέγγισή σου αν και θέλει πολύ κουβέντα ειδικά η φερριτίνη και τα ηπατικά στον ασυμπτωματικό ασθενή. Την φερριτίνη θα την ζητούσα μονο σε υπόχρωμη μικροκυτταρική αναιμία (ή ορθόχρωμη με πλακάκι ύποπτο για συνδυασμένη έλειψη Fe και B12 ή φυλικού) ή σε άτομο με συμπτωματική κόπωση ή σε υποψία αιμοχρωμάτωσης, ενώ για τα ηπατικά σε ασυμπτωματικούς ασθενείς θα πρέπει να ξέρουμε την επίπτωση ηπατικών νοσημάτων στον γενικό πληθυσμό και να μελετήσουμε το κόστος του screening σε σχέση με το κέρδος από μια πρώιμη διάγνωση/θεραπεία σε ένα τέτοιο φάσμα ασθενών για να αποφασίσουμε αν είναι cost effective. π.χ. πόσοι ασθενείς κάνουν ασυμπτωματική χολόσταση (για να μετράμε ALP) με νόσημα στο οποίο η πρώιμη διάγνωση παρέχει πρώιμη θεραπεία; PBC/PSC/καρκίνος παγκρέατος/χολάγγειοCa/φαρμακευτική κτλ...  (οι λιθιάσεις δεν παίζουν γιατί θα είναι συνήθως συμπτωματικές όταν επηρεάζουν την ALP). Αλλά αυτά είναι όλα νοσήματα με μάλλον μικρή επίπτωση στον γενικό πληθυσμό για να δικαιολογούν screening και το number needed to screen (NNS) για κάθε έτος ζωής κέρδος/ημέρα νοσηλείας κέρδος ...κτλ. μπορεί να εχει πολύ υψηλό κόστος. Ο τρόπος για να μειώσεις το NNS είναι να εστιάσεις σε ομάδες υψηλού κινδύνου αλλά και αυτό θέλει μελέτη.
Γενικά εγώ προτιμώ την προσέγγιση του USPSTF (Δεν είναι ορατοί οι σύνδεσμοι (links). Εγγραφή ή Είσοδος) και μετά τα guidelines των επιμέρους εταιριών.
Είναι εντυπωσιακό το πόσο συχνά όλοι αποφεύγουν να πάρουν θέση λόγω της έλλειψης evidence based δεδομένων.
Δυστυχώς ελλείψη ιατρικού φακέλου όπως έχουν στις σκανδιναβικές χώρες είναι πάρα πολύ δύσκολο να κάνουμε τέτοιες μελέτες στην Ελλάδα.
Σε τελική ανάλυση σημασία έχουν 4 πράγματα: το outcome, το κόστος, το πόσο είσαι διατεθημένος να πληρώσεις για κάθε ενέργεια που σου παρέχει ένα μετρήσιμο outcome και πόση ζημιά μπορεί να κάνεις με μια τέτοια πολιτική screening στους (μη)ασθενείς από τις εξετάσεις που ακολουθούν τα false positive (βλέπε PSA και διορθικό υπέρηχο με ΒΨ) ή στους καθυσηχασμένους πραγματικούς ασθενείς από τα false negative.
Μεγάλη κουβέντα ανοίξαμε αλλά χαίρομαι που υπάρχουν και άλλοι με τους ίδιους προβληματισμούς εκεί έξω.

Clinicians Often Underestimate Harms of Tests and Treatments.

Clinicians are likely to underestimate harms and overestimate benefits of tests and treatments, according to the results of a review of 48 studies published online January 9 in JAMA Internal Medicine.

"Patients cannot be assisted to make informed decisions if clinicians themselves do not have accurate expectations of intervention benefits and harms," write study authors Tammy Hoffman, PhD, and Chris Del Mar, MD, from the Centre for Research in Evidence-Based Practice at Bond University in Queensland, Australia.

The review showed that the majority of clinicians correctly estimated harms only 13% of the time, and benefits only 11% of the time. Previous studies on patient expectations show that they, too, overestimate benefits and underestimate harms of many aspects of their care.

The clinicians' estimates varied widely across specialties and treatments. For example, more than 90% overestimated hormone replacement therapy's ability to reduce the risk for hip fracture, whereas more than 90% underestimated the risk for fatal cancer from bone scans.

"This was a very nicely done systematic review," Daniel Matlock, MD, MPH, from the Department of Medicine at the University of Colorado School of Medicine, Denver, told Medscape Medical News. "Bottom line, this kind of highlights that doctors are human and subject to a lot of these same data biases that patients are as well."

The systematic review aimed to evaluate studies across all disciplines where clinicians were asked to estimate the benefits or harms of any test, screening, or treatment. The studies came from an initial pool identified from MEDLINE, EMBASE, CINAHL, and PsychINFO without restrictions on date, language, study design, or the study's references, for a total of 8166 papers. The researchers deemed 48 of the studies eligible for review. Of those, 20 covered treatments, 20 looked at medical imaging, and 8 addressed other tests or screening.

A total of 28 outcomes, across all the studies, compared clinicians' quantitative answers with an answer that was considered the "correct" one. The authors of the review did not attempt to verify whether each study's "correct" answer was actually the best, according to evidence at the time of publication.

The outcomes and responses were too variable to be combined into a meta-analysis, so instead, the researchers calculated the percentage of clinicians who underestimated, overestimated, or answered correctly about the benefits or harms in question.

As to the reasons for misperceptions of harms and benefits, the study authors speculate that clinicians may focus more on the mechanisms of tests and treatments than on the evidence for their effectiveness. They also suggest that it may simply be difficult to keep up with the evidence base, and that in some cases, journal articles and advertisements may be to blame for emphasizing positive aspects of interventions.

Clinicians' own biases may also include an enthusiasm for any treatment over none, or a desire for reassurance. There is also a proposed "therapeutic illusion," in which clinicians see interventions in a more positive light, especially ones they are more familiar with. One of the studies, which compared two specialties, found that clinicians were likely to think more highly of the intervention that they provided.

When the review authors looked just at medications, they found that clinicians overestimated both the benefits and the harms.

"I wonder if that's because we have more experience with [medications], and we tend to remember the bad situations," says Dr Matlock.

The authors speculate that the underestimation of harms from medical imaging procedures may be in part because harms occur long after the imaging occurs.

"Notwithstanding the challenges to doing so, addressing clinicians' distorted perceptions about the benefits and harms of screening, tests, and treatments is critical to optimal patient care," the authors write.

The authors and Dr Matlock have disclosed no relevant financial relationships.

Αν είναι έτσι στον Καναδά, πώς να είναι άραγε στην Ελλάδα...  :

   Μπα, δεν είναι ελληνικό σύμπτωμα, είναι διεθνές. Όπως φαίνεται βάζει μέσα και το treatment. Τα ιατρικά λάθη (και στη διάγνωση και στη θεραπεία) και οι υπερβολές είναι παγκόσμιο φαινόμενο. Προ εβδομάδος δεν ανέβασες το:Δεν είναι ορατοί οι σύνδεσμοι (links). Εγγραφή ή Είσοδος (αυτό στην Αυστραλία...).
   Δε νομίζω ότι είμαστε πολύ χάλια στην Ελλάδα. Ίσως μια (τεχνητή; ) υπερβολή σε CT - MRI.

Μία από τις μεγαλύτερες παρανοήσεις είναι το προβαλλόμενο ως δικαίωμα των ασθενών να κάνουν συχνότατα και αναίτια ένα πλήθος εξετάσεων, το οποίο βαφτίζουν ως "check-up", προσπαθώντας να κάνουν συμμέτοχο (και συνένοχο) το γιατρό σε αυτήν την πρακτική. Από την άλλη, όταν καλείς τους ίδιους να μετρήσεις μία πίεση ή να κάνεις μία βασική κλινική εξέταση, δείχνουν ως επί το πλείστον μεγάλη απροθυμία. Στον τομέα αυτό χρειάζεται παρέμβαση, όπου φυσικά ο ρόλος ενός οικογενειακού γιατρού είναι καταλυτικός.
Μελλοθάνατε ιατρέ, οι ασθενείς σου σε χαιρετούν.

Are We Ordering Too Many Labs?

Definitely more than necessary at teaching centers compared with nonteaching centers.

Anecdotal experience and small single-center studies suggest that physician trainees at teaching hospitals order more laboratory tests than do more experienced attending physicians. Researchers in Texas used a statewide database to compare mean number of laboratory tests per patient per hospital day among 43,000 adult inpatients with principal discharge diagnoses of bacterial pneumonia or cellulitis; patients were stratified by illness severity. Care settings were 11 major teaching hospitals, 12 minor teaching hospitals, and 73 nonteaching hospitals.

Laboratory tests per day differed significantly across all hospital types. After adjustment for length of stay and demographics, patients at major teaching hospitals underwent more testing than those at nonteaching hospitals for pneumonia (4 additional test orders per day) and cellulitis (3 additional test orders per day) across all levels of illness severity (except for the sickest patients with cellulitis). Although major teaching hospitals tended to admit sicker patients, researchers found no correlation between mean number of laboratory tests per day and mortality for either condition.

COMMENT

This study provides evidence that residency training environments are associated with a culture of more laboratory testing, without change in outcomes. Identifying the specific tests most responsible for these differences between teaching and nonteaching hospitals and the reasons for the differences would be helpful. A multimodal approach focused on education, provider audit and feedback, and electronic medical record–based order restriction has proven to be effective in limiting unnecessary laboratory ordering (JAMA Intern Med 2017; 177:1833).

What is an inappropriate or unnecessary test?  It can be defined as any test where the results are not likely to be medically necessary for clinical management of the patient. It should NOT be solely defined as any laboratory test unlikely to be reimbursed.

Examples of inappropriate tests include legitimate tests ordered in an inappropriate clinical setting. Examples include ordering PSA screening for men 75 years and older, placing daily orders for the same analyte in noncritical patients and screening the general population for vitamin D deficiency.

A second example is ordering laboratory tests that have weak evidence to support their clinical utility. Examples of these tests include IgG allergy tests,  Vertical Autoprofiler (VAP) for lipids, esoteric cardiovascular risk prevention profiles, expanded celiac disease panels and extensive patented neurologic panels.

A third example includes tests that represent pseudoscience lab profiles. Commonly marketed examples include oxidative stress analysis, estrogenomic profiles, detoxigenomic profiles , salivary hormone profiles for aging or menopause, and laboratory tests for chronic fatigue syndrome, fibromyalgia, autism, irritable bowel syndrome, chemical sensitivity and vaccine injury.

In this age of decreasing reimbursement, excessive ordering of laboratory tests should be considered a waste of medical resources.

The next blog will attempt to uncover the magnitude of unnecessary laboratory testing.
