Πολύς ο λόγος (και οι Ιατρικές μελέτες) τα τελευταία χρόνια που, και καλά, δείχνουν τις ευεργετικές επιπτώσεις του καφέ στην Υγεία.

Προσωπικά πιστεύω ότι αυτή η ιστορία με τα ευεργετικά οφέλη του καφέ θα ξεφουσκώσει με τα χρόνια όπως αυτή με τα ευεργετικά οφέλη του αλκοόλ. (αυτοί που αντέχουν να πίνουν καφέ ή αλκοόλ είναι απλώς πιο γερά σκαριά εξ αρχής, αλλά αρκετοί από αυτούς είναι και άνθρωποι της παρέας που από μόνο του έχει ευεργετική δράση, αυτό υποψιάζομαι εγώ).

Τα αποτελέσματα τέτοιων ερευνών, εννοείται ότι τα βλέπουν με πολύ καλό μάτι οι εταιρείες καφέ ή αλκοόλ, αν δεν τα επιδοτούν κιόλας (σε ορισμένες περιπτώσεις).

Τα 25 χρόνια μου ως κλινικός Ιατρός ΔΕΝ έχω δει σε ασθενείς οφέλη από τους καφέδες, αντίθετα έχω δει πολλούς να έχουν αϋπνίες, ευερεθιστότητα, και αρκετές ταχυκαρδίες ακόμη και καρδιακές αρρυθμίες. Δεν συζητάμε για τις γαστρίτιδες και τις γαστροοισοφαγικές παλινδρομήσεις καθώς και για τα κιλά που παίρνουν όσοι δεν φροντίζουν να πίνουν τον καφέ τους σκέτο.

Η επιπτώσεις επίσης στα προσωπικά οικονομικά μας είναι σοβαρές. Αν σε μία οικογένεια με 4 μέλη αγοράζει ο καθένας από απέξω 1 καφέ για 1,3 ευρώ/καφέ τότε στον χρόνο πάνω αυτή η οικογένεια θέλει 1.898 € μόνο για καφέδες…. Αφήστε στην άκρη το οικολογικό κόστος αν αγοράζεις τον καφέ σε πλαστικές συσκευασίες μιας χρήσης.

Η εξάρτηση βέβαια είναι μεγάλη. Έχω δει αρκετούς που προτιμούν χίλιες φορές να παίρνουν για πάντα πραζόλες (φάρμακα για το στομάχι) ή Eliquis (αντιπηκτικό φάρμακο που το δίνουμε σε καρδιακές αρρυθμίες γιατί τότε είναι αυξημένος ο κίνδυνος για Εγκεφαλικό), ακόμη και να κάνουν ablation στην καρδιά τους, παρά να δοκιμάσουν να κόψουν τον καφέ έστω και για λίγες εβδομάδες. ( «Ένας καφές μας έμεινε κτλ...» )

Πριν μερικά χρόνια διάβασα μια μελέτη (την παραθέτω παρακάτω) που κατέληγε ότι ο βέλτιστος αριθμός φλιτζανιών καφέ για να κατεβάσεις (μέσω του καφέ) τον κίνδυνο να αναπτύξεις Σακχαρώδη Διαβήτη είναι 6 φλυτζάνια/ημέρα !!!

Με λίγα λόγια, μας λέγανε αντί για νερό να πίνουμε καφέ.

Αν και δεν θεωρώ τον εαυτό μου ψεκασμένο, δεν μπορεί να μην πάει το μυαλό μου στο ότι ίσως να υπάρχουν από πίσω και συμφέροντα...

(Προσωπικά επειδή ξέρω το σώμα μου, γνωρίζω ότι αν έπινα 6 φλιτζάνια καφέ μερικές ημέρες στην σειρά θα κατέληγα στο Νοσοκομείο).

ABSTRACT
To evaluate the associations between coffee and caffeine consumption and various health outcomes, we performed an umbrella review of the evidence from meta-analyses of observational studies and randomized controlled trials (RCTs).
Of the 59 unique outcomes examined in the selected 112 meta-analyses of observational studies, coffee was associated with a probable decreased risk of breast, colorectal, colon, endometrial, and prostate cancers; cardiovascular disease and mortality; Parkinson's disease; and type-2 diabetes. Of the 14 unique outcomes examined in the 20 selected meta-analyses of observational studies, caffeine was associated with a probable decreased risk of Parkinson's disease and type-2 diabetes and an increased risk of pregnancy loss. Of the 12 unique acute outcomes examined in the selected 9 meta-analyses of RCTs, coffee was associated with a rise in serum lipids, but this result was affected by significant heterogeneity, and caffeine was associated with a rise in blood pressure. Given the spectrum of conditions studied and the robustness of many of the results, these findings indicate that coffee can be part of a healthful diet.

Ο καφές είναι μια απ΄τις ΑΠΟΛΑΥΣΕΙΣ αυτής της πρόσκαιρης ζωής. Σεβαστό να μην αρέσει σε κάποιους, αλλά ως επιστήμονες δεν μπορούμε να βασιζόμαστε σε πεποιθήσεις.
Οι μεταναλύσεις δείχνουν  ότι ασκεί ΕΥΓΕΡΓΕΤΙΚΗ επίδραση στην υγεία μας, ενώ βοηθά και στην απώλεια βάρους, άλλωστε συστήνεται η κατανάλωση χαπιών καφεϊνης πριν την προπόνηση στο γυμναστήριο για να επιταχυνθεί ο μεταβολισμός και να αυξηθούν οι καύσεις.
Απλά υπάρχουν ορισμένα όρια που πρέπει να σεβόμαστε, όρια γνωστά και δημοσιευμενα.

Για το οικονομικό, θα προσπεράσω. Προσωπικά παίρνω έναν απέξω κάθε μέρα και μου βγαίνει 60€ τον μήνα.
Είναι απ τα τελευταία χρήματα που θα κόψω στην ζωή μου....  :)

Most Paroxysmal AF Patients Report 'Triggers' Like Alcohol, Caffeine Intake.

Three-fourths of patients with symptomatic paroxysmal atrial fibrillation (AF) in a survey-based study identified at least one acute experience they believed was a trigger of individual episodes of their arrhythmia. The most commonly reported possible triggers were consumption of alcohol, caffeine intake, a bout of exercise, and a lack of sleep.

Among the nearly 1300 survey respondents, women and those with a family history of AF were the likeliest to report that they had such triggers and that they had multiple triggers. Those with heart failure in addition to symptomatic paroxysmal AF were among the least likely to report that their atrial arrhythmia had triggers.

One goal of the analysis was to identify potential AF triggers that are modifiable and could contribute to patient discussions and, in the case of family AF history, clarify interactions between genetic predisposition and environmental promoters, observed senior author Gregory M. Marcus, MD, MAS, University of California, San Francisco.

"It's a little unsatisfying," he told theheart.org | Medscape Cardiology, that "there appears to be quite a bit of heterogeneity regarding the perceived triggers, suggesting that, for example, a catch-all recommendation may not be appropriate."

For example, he noted, avoidance of caffeine seems to be an almost universal recommendation for patients with known paroxysmal AF.

"But the reality is, yes, there may be some people for whom caffeine is a trigger, but almost certainly many others for whom it is not. Same thing for exercise. We don't want to discourage exercise, but certain types of exercise may be an important trigger for some people," Marcus said.

"We need to better understand these more idiosyncratic relationships that may be most relevant to any one given individual."

The analysis was published February 14 in Heart Rhythm, with lead author Christopher A. Groh, MD, University of California, San Francisco.

Of the 1295 patients with symptomatic paroxysmal AF who responded to the survey, 74% reported experiencing at least one of the triggers on a provided list. Among that group, the most prevalent perceived triggers that were experienced "all or some of the time" were:

An occasion of drinking alcohol in 35%

Consuming caffeine in 28%

A bout of exercise in 23%

A lack of sleep in 21%

Respondents also were invited to "write in" perceived triggers not on the provided list. By far the most common write-in trigger, at 20% of respondents, was stress or anxiety.

The remaining triggers on the provided list, cited less often, included not exercising, drinking cold beverages, eating cold foods, eating a large meal, a high-sodium diet, dehydration, and lying on the left side

March said the provided list of possible triggers to which the patients responded were based on expert consensus or collected via online survey and by social media.

"There are almost no data to show that most of these things in fact trigger afib, so at this point the best evidence we have is what the patients are telling us."

The survey participants had been identified by being part of the Health eHeart Study or subscribers to the patient advocacy organization StopAfib.org.

Those who did and did not report having triggers for their AF episodes were statistically similar for demographics and cardiovascular comorbidities, except those reporting triggers were twice as likely to have a family AF history (P = .008) and 71% less likely to have heart failure (P = .001) in adjusted analysis.

Collectively, the respondents reported a median of two separate triggers. Groups most likely to report multiple triggers included women, younger patients, and those with Hispanic ethnicity, a family history of AF, or obstructive sleep apnea.

The findings are consistent with different mechanisms thought to be underlying AF in a broad range of patients, Marcus said. They include chronic, structural changes such as myocardial fibrosis or atrial enlargement, which track with advanced age and may be less associated with acute triggers.

Or they can include more dynamic and functional properties, such as ion-channel disturbance, that can be present in the absence of structural changes and may be more likely to react to environmental and behavioral triggers, he said.

In other words, triggers seem to be associated more with lone AF than AF with structural heart disease.

"It would make sense if you have an important environmental trigger that it would more likely influence something dynamic that could have an acute and transient effect on the electrical properties of the heart, as opposed to a kind of fixed structural issue that doesn't care whether alcohol is present or they didn't get enough sleep."

Marcus said the current analysis was part of preparation for the Individualized Studies of Triggers of Paroxysmal Atrial Fibrillation (I-STOP-AFib) randomized trial, the primary end point of which is based on the Atrial Fibrillation Effect on Quality of Life survey (AFEQT).

The trial plans to enter an estimated 478 smartphone-equipped patients with symptomatic paroxysmal AF, randomly assigning them to an intervention group or a control groups, both of which call for regular use of the Kardia Mobile (AliveCor) electrocardiographic monitor.

The intervention group, but not the control group, will make personalized lifestyle changes on the basis of a preceding period in which they tracked AF frequency, symptoms, and exposure to possible triggers. They'll conduct further monitoring to determine whether avoiding identified possible triggers has an effect on AF episodes.

"The goal is to be able to provide patients with their own information regarding the probability that a perceived trigger is in fact a trigger or not," Marcus said.

Marcus discloses research funding from Medtronic and Jawbone; consulting for Johnson & Johnson and InCarda Therapeutics; and holding equity in InCarda. Groh reported no conflicts. Disclosures for the other authors are in the report.

Results

We ascertained 4,311 and 2,730 incident AF cases in men and women, respectively, in the two cohorts. Coffee consumption was not associated with AF incidence in these cohort studies. The lack of association was confirmed in a meta-analysis, including six cohort studies with a total of 10,406 cases of AF diagnosed among 248,910 individuals. The overall relative risk (95 % confidence interval) of AF was 0.96 (0.84–1.08) for the highest versus lowest category of coffee consumption, and 0.99 (0.94–1.03) per 2 cups/day increment of coffee consumption.

Conclusions

We found no evidence that coffee consumption is associated with increased risk of AF.

Γείτονάς μου πάντως, που πέρυσι το καλοκαίρι είχε μια μέρα τα 3 από τα παρακάτω 4 (είχε πιει 1 απλό Ελληνικό καφέ, μετά από ένα 6ωρο περίπου 1 γαλλικό καφέ, δεν είχε κοιμηθεί καλά και κουράστηκε ανεβάζοντας κάτι βαλίτσες στον 3ο όροφο χωρίς ασανσέρ) έπαθε σε ηλικία 49 ετών Κολπική Μαρμαρυγή και έτρεχε στο Νοσοκομείο. Ευτυχώς ήταν παροδική. Από τότε ούτε που τον γλωσιάζει τον καφέ.

Με βάση την παραπάνω μελέτη που παρατέθηκε, δεν μπορεί με ασφάλεια να υπάρξει αιτιολογική συσχέτιση μεταξύ της κατανάλωσης καφεΐνης και της παροξυσμικής κολπικής μαρμαρυγής.
Αποτελεί δε προστάδιο μιας προοπτικής ,τυχαιοποιημένης μελέτης γι αυτό ακριβώς το θέμα, που ξεκίνησε πρόσφατα.Individualized Studies of Triggers of Paroxysmal Atrial Fibrillation (I-STOP-AFib).

   25 κούπες καφέ ημερησίως!!!! Ομολογώ εντυπωσιάστηκα....
   Το ζητούμενο από τα άνω δημοσιεύματα είναι αν προκαλεί ή όχι αρρυθμία ο καφές. Για αρτηριοσκλήρυνση δεν είχε αναφερθεί τίποτα. Τώρα γιατί οι Βρετανοί μπήκαν στον κόπο να το ελέγξουν (ας δουν μήπως προκαλεί και ...δισκοπάθεια) ποιος ξέρει.

   Το ζητούμενο από τα άνω δημοσιεύματα είναι αν προκαλεί ή όχι αρρυθμία ο καφές. Για αρτηριοσκλήρυνση δεν είχε αναφερθεί τίποτα. Τώρα γιατί οι Βρετανοί μπήκαν στον κόπο να το ελέγξουν (ας δουν μήπως προκαλεί και ...δισκοπάθεια) ποιος ξέρει.


Ακριβώς όπως τα λέει ο Ορθοπαιδικός.

Είναι γνωστό ότι ο καφές είναι αρρυθμιογόνος αλλά μπορεί να προκαλέσει και αυπνίες / νευρικότητα (για να μην μιλήσουμε για γαστρίτιδες και / ή γαστροοισοφαγική παλινδρόμηση).

Είναι τουλάχιστον ύποπτο ότι οι Βρεττανοί παρακάμπτουν αυτά και αντ' αυτών προσπαθούν να απαντήσουν σε ένα ερώτημα για το οποίο κανείς δεν έχει ενοχοποιήσει τον καφέ (αν δηλαδή ο καφές προκαλεί αρτηριοσκλήρυνση).

Από μελέτες που έχουν παρατεθεί παραπάνω πάντως, δεν προκύπτει ότι ο καφές προκαλεί αρρυθμίες.
