Tocilizumab for Patients with Severe COVID-19: The Confusion Continues

In a small Brazilian trial, tocilizumab did not improve clinical outcomes and might have increased mortality.

Elevated interleukin-6 (IL-6) levels are linked to worse outcomes in patients with COVID-19, but it remains unclear whether blocking this cytokine confers clinical benefit. Although large randomized trials have failed to show that tocilizumab, an anti–IL-6 receptor antibody, reduces mortality, some studies have suggested that the agent lowers risk for progression to mechanical ventilation (NEJM JW Infect Dis Jan 4 2021; [e-pub] and N Engl J Med 2020 Dec 17; [e-pub]) and shortens time to hospital discharge. Now, a small randomized trial in Brazil adds to the confusion.

In all, 129 patients requiring supplemental oxygen or mechanical ventilation who also had elevated inflammatory biomarkers were randomized to receive tocilizumab plus standard care or standard care alone. The trial was stopped early because of substantially higher mortality at day 15 in the tocilizumab group (17%) compared with the standard care group (3%). By day 28, the difference in mortality between the tocilizumab group and standard care group was no longer statistically significant (21% and 9%; P=0.07).

COMMENT
This small study did not demonstrate any benefit of tocilizumab and raised the possibility of increased mortality. Notably, standard care in this trial often included corticosteroids, but also drugs now known to lack benefit (e.g., hydroxychloroquine, azithromycin); remdesivir was not available. Larger trials have not shown excess risk for death or adverse events with tocilizumab, so the current study appears to be an outlier. We eagerly await publication of the REMAP-CAP trial results (preliminary findings from this large randomized trial indicate a reduction in mortality when tocilizumab is administered within 24 hours of initiating organ support in an intensive care unit). Whether tocilizumab has clinical benefit when given to the right patient at the right time is still being fiercely debated.

Η πρηνής (μπρούμυτα) θέση όσων είναι στην ΜΕΘ με COVID-19 ναι μεν έχει πλεονεκτήματα αλλά όμως αυξάνει και τον κίνδυνο για Meralgia Paresthetica.

Meralgia paresthetica after prolonged prone position at the intensive care unit among COVID-19 patients. A case report
Meralgia paresthetica (MP) is an entrapment syndrome that may cause loss of sensation, numbness, paresthesia and pain within the distribution of the lateral femoral cutaneous nerve. This condition is more common in persons with diabetes mellitus, obesity and in old age. MP has previously been described in patients that have undergone surgery in the prone position (PP) and in a case report of a patient with ARDS (Acute Respiratory Distress Syndrome) who was cared for in the intensive care unit (ICU). Due to the COVID-19 pandemic PP has been widely used for periods of 12-16 hours to improve oxygenation. At the rehabilitation unit at our hospital, we have identified cases of MP in patients with COVID-19 that have required this type of positioning for long periods in the ICU. We would like to draw attention to the fact that there is a risk of peripheral nerve injury in the event of prolonged PP and recommend extra controls, careful positioning and extra padding at the areas where peripheral nerves may be exposed to pressure.

Μια νέα μελέτη στο MMWR αναφέρει 21 περιπτώσεις αναφυλαξίας στις πρώτες 1.9 εκατ. δόσεις Pfizer που χορηγήθηκαν στις Ηνωμένες Πολιτείες. Αυτό μεταφράζεται σε 11.1 περιπτώσεις αναφυλαξίας ανά 1 εκατομμύρια δόσεις εμβολίου COVID. Για να το βάλουμε σε πλαίσιο, για εμβολιασμό γρίπης, το ποσοστό αναφυλαξίας είναι μόνο 1.3 ανά εκατομμύριο. Το εμβόλιο της Moderna έχει συνδεθεί και αυτό με την αναφυλαξία.

The First 30 Minutes: Anaphylaxis After COVID Vaccination

We now have two mRNA COVID vaccines.

For both, a two-dose series of the same vaccine is more than 90% effective. Pfizer/ BioNTech's mRNA COVID vaccine is FDA authorized for those 16 and older. Moderna's mRNA COVID vaccine is authorized for those 18 and older.

The phase 3 studies on both vaccines are huge: nearly 44,000 for the Pfizer vaccine and 30,000 participants for the Moderna vaccine. In these large trials, no cases of anaphylaxis were observed following administration of either vaccine. But remember the headlines about severe allergic reactions to Pfizer's vaccine in the UK? We're seeing them here in the US, too.

A new study in MMWR reports on 21 cases of anaphylaxis in the first 1.9 million Pfizer doses administered in the United States. This translates to 11.1 cases of anaphylaxis per 1 million COVID vaccine doses. To put this into context, for flu vaccination, the anaphylaxis rate is only 1.3 per million. Moderna's vaccine has been linked to anaphylaxis, too.

CDC's Dr Nancy Messonnier says that "even at 11 cases per million doses administered, it's a very safe vaccine... it's still a good value proposition for someone to get vaccinated."

Here's a head-to-head comparison of mRNA vaccine components. The ingredients for each are a little different (Table 1).

Each vaccine has four lipid ingredients that make up its protective lipid nanoparticle, forming a cushion around fragile mRNA to protect it and give it time to do its job.

Each vaccine also contains an array of salts, sugars, and buffers. Sugars keep the vaccine from clumping in the vial.

Neither vaccine contains eggs, gelatin, latex, or preservatives. Vaccine vial stoppers don't contain latex either.

Both vaccines contain polyethylene glycol (PEG).

Precautions and Contraindications
Here is CDC's guidance on COVID vaccines for patients with a history of allergies. This color-coded table provides a nice, quick-look summary (Table 2).

   Εγώ πάλι δεν μπορώ να καταλάβω γιατί πρέπει να κάνω το συγκεκριμένο εμβόλιο με πιθανότητα επιτυχίας 60%; Για ποιο λόγo να εγκριθεί όταν υπάρχουν άλλα καλύτερα (όχι μόνο τα 2 ήδη εγκεκριμένα αλλά και από άλλες χώρες που για κάποιους λόγους δεν προωθούνται).
   Με πιστοποιητικό εμβολιασμού (που κάποιοι σκέφτονται) από την ASTRA είναι πιο βέβαιο ότι δεν μεταδίδω ιό ή με φυσική νόσηση; Ή είναι πιο σίγουρο κάποιο πρόσφατο τεστ;

Τα άλλα δεν επαρκούν, αυτό υπάρχει, αυτό θα κάνουμε. 60% μικρότερη πιθανότητα νόσησης είναι καλύτερη από 0% μικρότερη πιθανότητα νόσησης αν δεν το κάνουμε.
Αν και βέβαια αυτά τα νούμερα έχουν βγει από σχετικά μικρές μελέτες με σπόνσορες ως επί το πλείστον τις εταιρείες που παρασκευάζουν το αντίστοιχο εμβόλιο...

   Θα συμφωνήσω για τα παραπάνω. Στους ασθενείς μου κάνω πχ έγχυση βηταμεθαζόνης σε τενοντοελυτρίτιδες (με επιτυχία ~50%) αντί για άμεσο χειρουργείο (με πιο σίγουρη επιτυχία) ή δίνω κάποια αγωγή που πιθανόν θα βοηθήσει....
   Εδώ όμως υπάρχει (πιστεύω) διαφορά. Ακόμα και αν εμβολιάσεις το 80% του πληθυσμού με το άνω εμβόλιο δεν έχεις ανοσία "αγέλης". Δεν ξέρεις ποιος είναι άνοσος, ποιος μεταδίδει κλπ... Και γιατί "εγώ" ως τρίτη χώρα να δεχτώ τον άνω εμβολιασμένο στη χώρα μου ξέροντας ότι είναι πολύ πιθανό (το 40% είναι πάρα πολύ) να είναι φορέας ή ασθενής (μήπως θα κάνουμε από την αρχή τέστ αντιγόνων και αντισωμάτων; ). Το βλέπω "μπάχαλο". Αφήστε που ο εμβολιασμένος θα έχει μία επιπλέον σιγουριά ότι όλα είναι εντάξει και δεν θα προσέχει πλέον...
   Θα υπάρχουν λοιπόν πατρίκιοι και πληβείοι και στα εμβόλια (όπως παντού τελικά)... Σαν λύση ανάγκης (όπως λέτε) να το δεχθώ (αν και μεγαλύτερη ανάγκη έχουν οι 65+ που δεν πήρε σαφή ένδειξη το εμβόλιο αυτό). Προσωπικά θα προσπαθήσω να το αποφύγω. Όποιος θέλει μπορεί να το κάνει βέβαια. Δεν είμαι αρνητής αλλά...
   Και, όταν υπάρχει ανάγκη, αφήνουμε την πολιτική και τις προκαταλήψεις και δεχόμαστε και από αλλού εμβόλια.

How Long Does Immunity Last After Recovery from Mild COVID-19?

Two studies suggest the adaptive immune response in COVID-19 is durable and continues to be shaped by viral remnants.

Our understanding of the immunity naturally conferred through SARS-CoV-2 infection is critical for gaining control of the COVID-19 pandemic. Immunologic memory is based on activation, expansion, and differentiation of B and T lymphocytes with viral neutralizing properties. With reexposure, these reactivated cell lines coordinate to clear the virus, preventing or mitigating disease and reducing transmission. Is this response durable in the setting of COVID-19?

Rodda et al. examined plasma and peripheral blood cells from 15 patients who recovered from mild COVID-19 (median duration of symptoms, 13 days) and 17 healthy controls. COVID-19 induced SARS-CoV-2–specific IgG antibodies, neutralizing plasma, and memory B and T lymphocytes that persisted for ≥3 months. Memory B lymphocytes expressed neutralizing antibodies against SARS-CoV-2, and memory T cells produced immune-activating cytokines (particularly IL-2 and interferon-gamma). The authors conclude that these manifold changes in the adaptive immune response render patients protected against SARS-CoV-2 infection for at least 3 months.

Gaebler at al. assessed humoral memory responses in 87 patients at both 40 days and 6 months after SARS-CoV-2 infection (median duration of symptoms, 12 days). Neutralizing IgG against the SARS-CoV-2 spike protein receptor binding domain (RBD) were detectable in plasma but decreased by 80% at 6 months compared with 40 days; however, the number of memory B cells with RBD specificity did not change. These cells evolved such that antibodies expressed at 6 months had increased neutralizing potency and breadth as well as resistance to RBD mutations. This evolution appeared to be driven by persistent viral antigens; indeed, a separate analysis of intestinal biopsies from convalescent asymptomatic patients showed SARS-CoV-2 nucleic acids and antigens in the small bowel of 7 of 14 patients. The authors conclude that viral remnants continue to drive adaptive immune evolution, optimizing the humoral response over time.

COMMENT
These studies provide in-depth analysis of the adaptive immune response to SARS-CoV-2, demonstrating residual humoral and cellular immune protection lasting at least 3 months as well as ongoing immune stimulation that continues to shape the host response long after clinical infection. These findings suggest that reinfection in immunocompetent individuals may be less likely than was initially feared. While viral antigen persistence may underlie long-lasting immunity, and perhaps preserve protection against SARS-CoV-2 variants, this protection may come at the price of “long-hauler” patients with persistent symptoms of COVID-19 driven by continued immune stimulation.

At the time we reviewed the paper by Gaebler et al., its publisher noted that it was not in final form and that subsequent changes might be made.

"The Pfizer/BioNTech and Moderna mRNA vaccines contain polyethylene glycol (PEG). PEGs (also known as macrogols) are a group of known allergens commonly found in medicines, many household products and cosmetics. Medicines containing PEG include some tablets, laxatives, depot steroid injections, and some bowel preparations used for colonoscopy. Known allergy to PEG is rare but would contraindicate receipt of this vaccine. (Sellaturay P et al, 2020). It is unclear whether PEG is the only cause of allergic reactions in patients with systemic allergic symptoms after the first dose of Pfizer/BioNTech vaccine.
The rate of anaphylaxis reported to date to the AstraZeneca vaccine is in line with the expected rate of anaphylaxis to non-COVID vaccines. The AstraZeneca vaccine does not contain PEG but does contain a related compound called polysorbate 80. Some people with PEG allergy may also be allergic to polysorbate 80. However, polysorbate 80 is widely used in medicines and foods, and is present in many medicines including monoclonal antibody preparations. Some injected influenza vaccines (including the main vaccine used in over 65 year olds) contain polysorbate 80. Individuals who have tolerated injections that contain polysorbate 80 (such as certain influenza vaccines) are likely to tolerate the AstraZeneca vaccine. This is summarised in table 4."

Σε συνάρτηση με τα παραπάνω έχουν προκύψει όμως και κάποια αλλα στοιχεία, δείχνοντας ότι η vitD εχει θέση στην πρόληψη της σοβαρής COVID-19 νόσου και όχι τόσο στη θεραπεία αυτής.

Vitamin D deficiency markedly increases the chance of having severe disease after infection with SARS Cov-2. The intensity of inflammatory response is also higher in vitamin D deficient COVID-19 patients. This all translates to increase morbidity and mortality in COVID-19 patients who are deficient in vitamin D. Keeping the current COVID-19 pandemic in view authors recommend administration of vitamin D supplements to population at risk for COVID-19.
