Epidemiology of Long-Term COVID-19 Symptoms

Nearly three quarters of patients had at least one persistent symptom following SARS-CoV-2 infection.

Studies of patients who have recovered from SARS-CoV-2 infection but have persistent symptoms, so called “long-haulers,” have ranged widely in size, quality, and methodology, leading to confusion about the prevalence and types of persistent symptoms. In this meta-analysis, researchers identified nearly 2000 studies that addressed persistent post–COVID-19 symptoms; 45 studies, with a total of ≈10,000 patients and 84 discrete symptoms, were deemed suitable for analysis. The studies were highly heterogeneous, including wide variation in the severity and nature of the original infections.

Nearly three quarters of patients reported at least one persistent symptom for a mean or median of 2 to 4 months; and some symptoms were reported to persist for as long as 6 to 8 months. The most common symptoms were fatigue (40% of patients), shortness of breath (36%), anosmia (24%), anxiety (22%), persistent cough (17%), ageusia (16%), and depression (15%).

COMMENT
This systematic analysis really constitutes more of a large case series than a robust meta-analysis because of the highly heterogeneous nature of the original studies. The results will help clinicians advise patients about what to expect post-COVID, albeit without much guidance on specific treatments.

In a Large Randomized Trial, Convalescent Plasma Strikes Out

Among patients hospitalized with COVID-19, convalescent plasma did not improve survival over usual care.

From COVID-19's earliest days, patients have been treated with plasma from individuals who have recovered from their infections (convalescent plasma; CP). Still, it remains unclear whether this therapy improves outcomes. Investigators in the U.K. randomized 11,558 hospitalized participants in the RECOVERY trial to receive open-label, high-titer CP plus usual care or usual care alone. Median time from symptom onset to randomization was 9 days. Regarding disease severity at enrollment, 87% of participants required oxygen only, 5% were receiving invasive mechanical ventilation (IMV), and 8% did not need any supplemental oxygen. Use of other therapies, such as corticosteroids and remdesivir, was balanced between the two groups.

Mortality at 28 days was 24% in both the CP and usual care groups. Time to discharge, probability of being discharged alive, and number of patients progressing to IMV or death were similar in both groups. Among participants negative for anti–SARS-CoV-2 antibodies at baseline, mortality was comparable in the CP and usual care groups (32% and 34%, respectively).

COMMENT
As this large, randomized trial does not demonstrate a survival benefit of CP in hospitalized patients with COVID-19, the findings should steer us away from such treatment in this setting. However, the study does not answer whether convalescent plasma would improve outcomes among high-risk patients earlier in the course of their disease (as suggested in a different study; NEJM JW Infect Dis Mar 2021 and N Engl J Med 2021 Jan 6; [e-pub]) or in highly immunosuppressed patients with persistent SARS-CoV-2 replication. Those important patient populations require evaluation in future studies.

Τι γίνεται επίσημα με τον εμβολιασμό όσων έχουν φαρμακευτικές αλλεργίες; νομίζω στα εμβολιαστικά κέντρα αυθαίρετα διώχνουν τον κόσμο. Με προβλημάτισε πολύ μια ασθενής μου που με πήρε πολύ αγχωμένη γιατί την τρόμαξαν χωρίς λόγο. Μπορώ να δώσω γνωμάτευση εγώ ή χρειάζεται υποχρεωτικά αλλεργιολόγος, όπως της είπαν; υπάρχει κάποια οδηγία;

Therapeutic Doses of Anticoagulation Did Not Benefit Patients with COVID-19 and Elevated D-Dimer Levels

In an open-label trial involving hospitalized patients, therapeutic anticoagulation dosing did not improve overall outcomes and was associated with more bleeding than prophylactic anticoagulation.

Compared with other respiratory infections, COVID-19 is associated with higher incidence of thromboembolic events — moreover, these complications are thought to be more likely in patients with elevated D-dimer concentrations. Observational studies have suggested that both therapeutic and prophylactic doses of anticoagulation may reduce risk for in-hospital death in such patients, but controlled studies are lacking. To address this question, researchers conducted an open-label trial at 31 Brazilian hospitals involving patients hospitalized with COVID-19 who had elevated D-dimer levels; patients with diagnosed venous thromboembolism (VTE) were excluded. Among 615 participants, 304 were randomized to standard prophylactic anticoagulation with heparin during hospitalization and 311 to therapeutic doses of anticoagulation with either rivaroxaban or heparin for 30 days.

Time to death, duration of hospitalization, and duration of supplemental oxygen through 30 days were not statistically significantly different between groups; nor was there a difference in incidence of individual thrombotic events at 30 days. However, clinically relevant bleeding occurred in 26 patients (8%) receiving therapeutic doses of anticoagulation and 7 (2%) receiving prophylactic anticoagulation (P=0.001).

COMMENT
Based on this well-designed study in patients with elevated D-dimer levels, there is no indication for using therapeutic anticoagulation dosing for VTE prophylaxis in patients with COVID-19. The results support current guidance and do not change practice.

Επιβεβαιώνει τις εκτιμήσεις για επικείμενη επικράτηση
του και στην Ελλάδα
Αυξητική τάση εμφάνισης του στελέχους Δέλτα του SARS-Cov-2, στα λύματα της
Θεσσαλονίκης, αναφέρει η έρευνα της Ομάδας Επιδημιολογίας Λυμάτων του ΑΠΘ
και του Ινστιτούτου Εφαρμοσμένων Βιοεπιστημών του ΕΚΕΤΑ.
Η εικόνα αυτή επιβεβαιώνει τις εκτιμήσεις για αναμενόμενη -σε μικρό χρονικό
διάστημα- επικράτηση του ινδικού στελέχους και στην Ελλάδα, όπως συνέβη
και σε πολλές χώρες. Πάντως μέχρι σήμερα, η παραλλαγή του κορονοϊού που
επικρατείσ στα λύμετα είναι η Βρετανική. Σημειώνεται, πως ο εντοπισμός του
στελέχους Δέλτα έγινε την περίοδο από 4 έως 24 Ιουνίου, αφορά δηλαδή μία
περίοδο με χαμηλό ιικό φορτίο στα λύματα, αλλά και μικρό ημερήσιο αριθμό
των επιβεβαιωμένων από τον ΕΟΔΥ κρουσμάτων. Η ανίχνευση κατέστη εφικτή
μέσα από μεθοδολογία συμπύκνωσης και καθαρισμού του γενετικού υλικού
του SARS-CoV-2 από μεγάλη ποσότητα λύματος, καθώς και αξιολόγησης της
ποσότητας και καθαρότητας του για την περαιτέρω μοριακή ανάλυση. «Η Ομάδα
Επιδημιολογίας Λυμάτων του ΑΠΘ σε συνεργασία με το ΕΚΕΤΑ εργάζεται εδώ
και μήνες στη μελέτη των μεταλλάξεων στα λύματα. Η προσπάθεια αυτή σε
σχέση με τη μέτρηση ιικού φορτίου στα λύματα, είναι εξίσου πρωτοπόρα αλλά
και δύσκολη, επειδή δεν περιορίζεται απλά στην ανίχνευση αλλά προχωράει
ένα βήμα παραπέρα στον ποσοτικό προσδιορισμό των μεταλλάξεων, έτσι ώστε
να μπορεί να παρακολουθείται η ένταση της παρουσίας των μεταλλαγμένων
στελεχών σε όλη την κοινότητα», δήλωσε στο ΑΠΕ-ΜΠΕ ο πρύτανης του ΑΠΘ και
επιστημονικά υπεύθυνος του ερευνητικού έργου, καθηγητής Νίκος Παπαϊωάννου.

Δημοσιεύτηκαν στο περιοδικό Nature

Οι Ιατροί της Θεραπευτικής Κλινικής της Ιατρικής Σχολής του Εθνικού και
Καποδιστριακού Πανεπιστημίου Αθηνών, Θεοδώρα Ψαλτοπούλου, Πάνος
Μαλανδράκης, Γιάννης Ντάνασης και Θάνος Δημόπουλος (Πρύτανης ΕΚΠΑ)
συνοψίζουν τις δέκα συστάσεις για την COVID-19 λοίμωξη όπως δημοσιεύτηκαν στο
έγκριτο διεθνές περιοδικό Nature.

Για το κοινό προτείνεται:

1. Να χρησιμοποιείται μάσκα σωστά τοποθετημένη, πάντα σε δημόσιους χώρους. Οι
Ν95 μάσκες και η διπλή χειρουργική μάσκα φάνηκαν πιο αποτελεσματικές από την
απλή μάσκα σε μία μεταανάλυση από ένα σύνολο 39 μελετών, που συμπεριέλαβε
πάνω από 12.000 άτομα.

2. Να αποφεύγονται οι πολυσύχναστους χώροι, ιδίως εσωτερικοί. Σε δεδομένα από
38 μελέτες παρατήρησης φάνηκε ότι ο κίνδυνος μετάδοσης μειώνεται σημαντικά
όταν η τηρείται απόσταση τουλάχιστον ενός μέτρου.

3. Υποβολή σε τεστ με συμπτώματα της νόσου COVID-19 και απομόνωση με ήπια
συμπτώματα, όπως εμπύρετο, φαρυγγαλγία, βήχα, απώλεια γεύσης ή όσφρησης.
Οι περισσότεροι ασθενείς μπορούν να αναρρώσουν σπίτι μόνο με τακτική
παρακολούθηση της θερμοκρασίας και του κορεσμού, με μόνη παρέμβαση την
επαρκή ενυδάτωση και τη χρήση παρακεταμόλης για το εμπύρετο και τις μυαλγίες.

Δεν ενδείκνυται κανένα άλλο φάρμακο.

4. Να κληθεί γιατρός αν εμφανιστεί δύσπνοια ή αν ο κορεσμός του οξυγόνου πέσει
κάτω από 92%, ενώ η πρηνής θέση φαίνεται ότι προσωρινά διευκολύνει την
αναπνοή.

5. Εμβολιασμός όσο το δυνατόν πιο άμεσα, ακόμα και αν κάποιος έχει νοσήσει με
λοίμωξη COVID-19.

Για τους λειτουργούς υγείας:

Να μην χρησιμοποιούνται μη αποδεδειγμένες ή αναποτελεσματικές θεραπείες
για τη λοίμωξη COVID-19. Δεν υπάρχουν επαρκή δεδομένα να στηρίξουν
την αποτελεσματικότητα των ακόλουθων φαρμάκων: favipiravir, ivermectin,
azithromycin, doxycycline, oseltamivir, lopinavir-ritonavir, hydroxychloroquine,
itolizumab, bevacizumab, IFN-a2b, fluvoxamine, convalescent plasma, και δεν
συνιστώνται από τον Παγκόσμιο Οργανισμό Υγείας.

7. Να μην χρησιμοποιούνται φάρμακα όπως το remdesivir και το tocilizumab, εκτός
από τις περιστάσεις όπου όντως ενδείκνυνται. Το tocilizumab ενδείκνυται σε
ασθενείς με σοβαρή νόσο που ήδη λαμβάνουν αγωγή με κορτικοστεροειδή, που
έχουν συστηματική φλεγμονώδη αντίδραση και ταχέως αυξανόμενες ανάγκες
σε οξυγόνο. Το remdesivir βραχύνει αποδεδειγμένα το χρόνο έως την ίαση των
ενηλίκων όταν χορηγείται νωρίς στη νόσο σε κάποιους ασθενείς σε κλινικές
μελέτες, που χρήζουν υποστήριξης με οξυγόνο.

8. Να χρησιμοποιούνται τα κορτικοστεροειδή με σύνεση σε ασθενείς με
υποξυγοναιμία, παρακολουθώντας στενά τις τιμές του σακχάρου αίματος για να
διατηρούνται στα φυσιολογικά όρια.

9. Δεν χρειάζεται να προγραμματίζονται τακτικά απεικονίσεις όπως αξονικές
τομογραφίες ή δείκτες φλεγμονής, όπως η φερριτίνη, η IL-6, η LDH και η
προκαλσιτονίνη.

10. Πρέπει να δίνεται η πρέπουσα σημασία και σε άλλες σοβαρές παθήσεις όχι
σχετιζόμενες με την COVID-19.

Μεταξύ των εξωτερικών ασθενών με μόλυνση από #SARSCoV2, η θεραπεία με μία μόνο δόση αζιθρομυκίνης σε σύγκριση με εικονικό φάρμακο δεν είχε ως αποτέλεσμα μεγαλύτερη πιθανότητα να είναι απαλλαγμένο από συμπτώματα την ημέρα 14.
