Αυτό το θέμα ήταν κάτι που με είχε βάλει σε αμφιβολίες πολύ καιρό. Μάλιστα, είχα εισπράξει και την απορία από έναν ειδικευόμενο γιατί δεν στέλνουμε PSA βάση των οδηγιών...
Τελικά, εμπειρικά και θα ήθελα να το μοιραστώ μαζί σας ακολουθώ την εξής ταχτική:
Ρωτάω τον ασθενή εάν το τελευταίο εξάμηνο έχει παρατηρήσει κάποια αλλαγή στον τρόπο που ουρεί (συχνότητα, ακτίνα ούρων, άλλες ενοχλήσεις). Εάν έχει οτιδήποτε από αυτά στέλνω άμεσα PSA. Εάν υπάρχει κληρονομικό ιστορικό είμαι πιο επιρρεπής να στείλω PSA.
To πρόβλημα είναι συνήθως οι ηλικιωμένοι που αρχίζουν να μην έχουν μνήμη σε βάθος εξαμήνου αλλά εκεί συνήθως δεν θα πεθάνουν από αυτό....

Conclusions: No single cut-off value for prostate specific antigen concentration attained likelihood ratios formally required for a screening test. Prostate specific antigen concentrations below 1.0 ng/ml virtually ruled out a prostate cancer diagnosis during the follow-up. Additional biomarkers for early detection of prostate cancer are needed before population based screening for prostate cancer should be introduced.

the American Urological Association released its second Choosing Wisely list, which called on physicians to “offer PSA testing for detecting prostate cancer only after engaging in shared decision making.” Not a complete about-face, but I give them credit for not dodging the issue this time around.

Primary Care Physicians and Urologists Respond Differently to Recommendations Against PSA Screening.

Use of prostate-specific antigen testing declined dramatically among primary care physicians.
Controversy is ongoing about use of prostate-specific antigen (PSA) testing to screen for prostate cancer. In late 2011, the U.S. Preventive Services Task Force (USPSTF) issued a recommendation against PSA screening for all men (NEJM JW Gen Med Jul 1 2012 and Ann Intern Med 2012; 157:120). To assess the effect of this recommendation on PSA testing frequency, researchers used a national ambulatory care survey to identify 2300 men (age range, 50–74) who visited primary care physicians or urologists for preventive services in 2010 and 2012 (i.e., before and after the recommendation was issued). Men with any prostate or previous PSA abnormalities were excluded.
Between 2010 and 2012, use of PSA testing fell significantly, from 37% to 16%, among men who visited primary care providers. Testing rates decreased from 39% to 35% among men who saw urologists.
COMMENT
Primary care physicians and urologists appear to have responded differently to the USPSTF recommendation against PSA screening, possibly because of differences in their patient populations, their experience with prostate cancer, their interpretation of the data that led to the recommendation, and conflict between the USPSTF guideline and the American Urological Association guideline that was in effect in 2010 and 2012 and favored screening. In my 40 years of clinical practice I have had less than 10 men die of metastatic prostate cancer. Probably around 20 men with serious complications of prostate biopsy--bleeding, sepsis. Dozens with disabling GU symptoms. Very difficult call.

Το πρόβλημα με τον καρκίνο του προστάτη είναι ότι δεν μπορούμε να ξεχωρίσουμε ποιος από όλους θα προχωρήσει και ποιος όχι.
Το Gleason score δίνει μια ένδειξη για την επιθετικότητα όταν είναι υψηλό.
Ένα 3+3 ή ένα 3+4 μπορεί να συνοδεύει τον άντρα για δεκαετίες χωρίς να τον σκοτώνει, ένα μικρό ποσοστό όμως από αυτούς μπορεί και να δώσει μετάσταση μέσα σε λίγα χρόνια.
Η ιστολογική εικόνα είναι η ίδια και δεν έχουμε κάποιο αξιόπιστο γενετικό δείκτη που να φανερώνει την επιθετικότητα.
Προσωπικά θα επέλεγα να κάνω ένα PSA μετά τα 55, βιοψία αν τα απεικονιστικά και η κλινική εξέταση συνηγορούσαν, ενεργή παρακολούθηση στην περίπτωση θετικής απάντησης/εφόσον δεν είχα gleason 5 και τα απεικονιστικά βοηθούσαν.
Μέτα τα 70 δεν θα έκανα τίποτα ούτε PSA. Αυτά είναι προσωπική μου άποψη και ΔΕΝ είμαι ουρολόγος.

Οι περισσότεροι άνδρες στην Ελλάδα μετά τα 50, μετά την χοληστερίνη "για να δουν που βρίσκονται" (αφού έχουν σαβουρώσει ό,τι κρέας υπάρχει), ζητούν να τους γράψεις και ένα PSA...
