Swimming induced pulmonary edema

This article describes the case of a previously healthy 23-year old male who developed swimming induced pulmonary edema (SIPE) during a 250 km swimrun race in Stockholm in September 2025. After 15 hours of combined running and swimming he developed dyspnea, cough, pink sputum, saturation of 65% on air, and crackles on lung auscultation. The patient was airlifted to the Karolinska University Hospital in Solna, where he received BiPAP treatment. An echocardiogram revealed no abnormalities apart from a discreet tricuspid regurgitation. The patient was discharged after 24 hours.

In this article we briefly describe pathophysiology and diagnostic criteria. Our goal with this case report is to inform and alert physicians of this relatively rare condition, as we’ve noted a marked increase of patients presenting with SIPE in parallel to an increase in swimrun race participation in recent years.
