Το T. Diuren 25 mg x 20 παρουσιάζει έλλειψη.

Μη σαμποτάρετε τα βασικά και φθηνά φάρμακα.

Μπορούμε να αντικαθιστούμε 25mg υδροχλωροθειαζίδης με 12,5mg χλωροθαλιδόνης (Hygroton).

The therapeutic dose ranges of hydrochlorothiazide, chlorthalidone, and bendroflumethiazide monotherapy are stated to be 12.0 to 50.0, 12.0 to 25.0, and 2.5 mg, respectively.19,20 From Figure 3A and 3B, it can be seen that these dose ranges are not equieffective for BP reduction; for systolic BP the equivalent dose of hydrochlorothiazide 25.0 mg is ≈8.0 mg of chlorthalidone and 1.5 mg of bendroflumethiazide. A common reference source21 states that hydrochlorothiazide and chlorthalidone are equipotent, although in vitro studies suggest that the affinity of chlorthalidone for the NaCl cotransporter is ≈2-fold greater than hydrochlorothiazide and that bendroflumethiazide has a 20-fold greater affinity for the NaCl cotransporter than hydrochlorothiazide.6 In addition, the half-life of chlorthalidone is considerably longer than hydrochlorothiazide.

Άρα τα 12,5 mg Diuren πρέπει να αντιστοιχούν σε 4 mg Hygroton.

Να πούμε ότι όπως το Diuren έτσι και το Hygroton έχει χαραγή διχοτόμησης, αλλά στην πράξη μπορούν και τα δύο φάρμακα και να τετρατομηθούν.

Άρα τα 12,5 mg Diuren πρέπει να αντιστοιχούν σε 4 mg Hygroton.

Να πούμε ότι όπως το Diuren έτσι και το Hygroton έχει χαραγή διχοτόμησης, αλλά στην πράξη μπορούν και τα δύο φάρμακα και να τετρατομηθούν.

Άραγε οι αντιστοιχίες αυτές ισχύουν, εκτός από την δραστικότητα στην μείωση της Αρτηριακής Πίεσης, και στην πιθανότητα για υποκαλιαιμία;

 Παραθετω μια ενδιαφερουσα ανασκοπηση του NEJM για τη χρηση των διουρητικων στην υπέρταση. Φαινεται οτι το Hygroton ειναι μάλλον καλύτερο απο την υδροχλωροθειαζίδη στην 24ωρη ρύθμιση της πίεσης. Επίσης όσον αφορα τη θεραπευτικη αντιστοιχια τους γραφεται οτι  "...chlorthalidone is approximately twice as potent as hydrochlorothiazide".

Απ,ότι φαίνεται σε γράφημα του παραπάνω άρθρου τα 25mg HCTZ και τα 12,5 χλωρθαλιδόνης επιφέρουν παρόμοια μείωση των επιπέδων Κ κατά 0,3-0,4 mmol/ L.
